Requirements

Reporting directly to Aging Care Management Supervisor 1, a Care Manager 2, within the Luzerne and Wyoming Counties Area Agency on Aging (AAA) will assess the needs of consumers utilizing state-mandated assessment forms.

This position develops effective care plans to meet the needs of their consumers and implements these care plans in a professional manner.

This role conducts field visits in order to determine a level of care for the consumer.

Maintaining a knowledge of all AAA service programs as well as available community services is essential for carrying out duties.

You will manage phone requests for services, collecting information and providing the appropriate referral for callers.


Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
  • Telework: You will not have the option to telework in this position.
  • Salary: In some cases, the starting salary may be non-negotiable.

Qualifications

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or

  • Hiring preference for this vacancy may be given to candidates who live within either Luzerne or Wyoming County. If no eligible candidates who live within either Luzerne or Wyoming County apply for this position, candidates who reside in other counties may be considered.
