The Luzerne/Wyoming Counties Area Agency on Aging is seeking a motivated and compassionate individual to join our team as an Aging Care Manager 1.
This is a six-month probationary training position designed to equip you with the knowledge and skills needed to support older Pennsylvanians in maintaining independence, dignity, and quality of life.

The Aging Care Manager must possess the knowledge, skills, and abilities necessary to effectively serve older adults following completion of required training.
The employee must be able to recognize and appropriately respond to consumers who may be at risk, including situations involving health, safety, neglect, or exploitation.
Strong oral and written communication skills are required to interact professionally with consumers, caregivers, service providers, and colleagues.
The position requires the ability to operate a computer, cell phone, and standard office equipment, along with working knowledge of Microsoft Office applications.
The Aging Care Manager must be able to organize, prioritize, and manage a diverse caseload while meeting established timelines and documentation standards.
The employee must interpret and apply relevant laws, regulations, policies, and program directives in the performance of assigned duties.
The position requires independent travel throughout the service area, including areas that may not be accessible by public transportation.
Additionally, the employee must be able to analyze consumer information, data, and supporting documentation to determine eligibility and develop appropriate care plans.
Responsibilities include preparing reports, maintaining accurate and timely case documentation, and delivering presentations as needed.
The Aging Care Manager must also remain current on issues, programs, and resources impacting older Pennsylvanians and, upon completion of training, accurately record and maintain all case activity in compliance with agency procedures and state requirements.
