What You Will Be Doing:
This position serves as the subject matter expert on advancing measurable improvements in population health outcomes through a Quality Improvement (QI) approach.
The position provides strategic and technical expertise to help clinics, community-based organizations, and state partners strengthen implementation of evidence-based interventions using proven QI methods, independently adapting and applying established theories, standards, and models to fit varied organizational and community contexts.
This position possesses knowledge to work across the continuum of quality improvement initiatives related to cardiovascular health (defined as heart health, diabetes prevention and management and brain health) including: advancement of health information technology (HIT) ande-referral infrastructure, patient access to prevention and management and social support services delivered in community or clinical settings, and health system implementation through a quality improvement approach.

A combination of related education, graduation from an accredited college or university in public health, health promotion, health policy, healthcare administration, epidemiology, health sciences, social work, nursing or a field of study closely related to the work assignment and professional experience in public health, health systems improvement, healthcare quality improvement, evidence-based intervention implementation, evidence-based disease management and/or prevention in public health or in a clinic or other health-related organization.
This includes supporting team-based care models, clinical workflow development, and using data to inform improvement strategies equal to Six (6) years.

In addition to the minimum requirements, the successful candidate in this position will have the following experience:
CONDITIONS OF EMPLOYMENT
TRAVEL: 10% of time; travel is predominately in-state to meet with local stakeholders, partners and grantees, with occasional out-of-state travel to meet with national partners.
Local travel is approximately 2-5 times monthly; out of state travel is approximately two times per year.
Travel is predominately in the metro Denver area with some occasional travel to rural Colorado; out of state travel is most often to conferences or national meetings and the location varies.
