Job Description

Summary

This job involves helping seniors and adults with disabilities by assessing their needs, creating personalized care plans, and coordinating in-home services to support their independence.

A good fit would be someone with a background in social services or health who enjoys working directly with clients and has experience in care coordination.

It's a full-time role in Sequim, Washington, focused on compassionate, hands-on support.

Key Requirements

  • Master’s Degree in behavioral or health sciences plus 1 year of paid social service experience, or Bachelor’s Degree in similar fields plus 2 years of paid social service experience, or Bachelor’s Degree plus 4 years of paid social service experience
  • Ability to assess care needs for seniors and adults with disabilities
  • Skills in creating service plans and coordinating care providers
  • Focus on promoting client independence through in-home care
  • Current Washington Driver's License
  • Access to an insured vehicle for work-related travel
  • Commitment to full-time, 40 hours per week non-exempt schedule

CASE MANAGER for Olympic Area Agency on Aging (O3A) based in Sequim, WA.


Requirements

Provides case management for in-home care to seniors and adults with disabilities including assessment of care needs, creating service plans, coordinating care providers, and focusing on client independence.

Qualifications

Master’s Degree in behavioral or health sciences & 1 yr. paid social service experience or BA in similar fields and 2 yrs. paid social service experience; or BA and 4 yrs. paid social service experience.


Requires current WDL & insured vehicle.

Additional Information

40 hrs./wk. Non-Exempt.


$68,401 to $89,749 annually.
