Snohomish County Human Services Department currently has one (1) FLSA Non-Exempt Registered Nurse position in the Aging & Disability Services Division.
Purpose:
Snohomish County Human Services Case Management is hiring a registered nurse to provide clients, caregivers and case managers nursing services to enhance the development and implementation of the client's care plan.
The nurse will offer consult for adult clients with unstable or chronic health conditions who are at risk of hospitalization or residential placement. This is a hybrid position.
About You:
You have clinical experience and can function independently using critical thinking skills and the nursing process to carry out job responsibilities.
You enjoy working with disabled and geriatric clients in the home setting and are comfortable providing education and training for clients, caregivers, and case managers.
Position Purpose:
To work in collaboration with case managers, caregivers, and members of the client’s health care team to review care needs and address concerns.
There is no direct nursing care involved that requires physician orders.
The case management nurse assists with developing and implementing the care plan and provides education and resource coordination to facilitate an optimal level of functioning and health for the client enabling them to remain safely in their home.

Responds to referrals from non RN Case Managers by providing file reviews of the CARE assessment, other client related care plans, and medical information; office consultation or staffing with case managers regarding client specific disease processes, diagnoses, & medications; telephone consultation with clients, providers, or case managers; and home visits to clients.
RNs complete documentation related to file reviews, consultations, and home visits.
Provides care coordination for clients through consultation and coordination with all pertinent members of a client's care team and facilitates health-related referrals to other services and programs as needed.
The RNs provide education regarding available community resources and programs related to the health care needs of the clients.
MINIMUM QUALIFICATIONS
Graduation from an accredited school of professional nursing; AND two (2) years of paid on-the-job nursing. Must pass job related tests.
PREFERRED QUALIFICATIONS
A Bachelor's degree in nursing.
