Full Job Description

The Nashville Strong Babies (NSB) project is seeking a motivated and driven individual to act as our Fatherhood Engagement Care Manager. This position works on the front lines of the NSB program, educating fathers and male partners about the critical role they play in nurturing and protecting their newborn children. Our Fatherhood Engagement Care Manager will assist and support male role models as important partners within our NSB- enrolled families and will work diligently to connect male parents with appropriate resources. This position is housed in the MPHD Population Health Bureau and requires some weekend and after-hours availability. This individual will report to the Nashville Strong Babies Program Manager.

Requirements

  • Works directly with the male partners/fathers of NSB enrollees and potential enrollees in the target area through daily outreach, recruitment and enrollment of men into NSB.
  • Manages assigned caseloads of male/father participants using advanced problem-solving and critical thinking to assist clients in navigating the NSB program.

Maintains direct contact with NSB enrollees and potential enrollees in the target area; includes conducting daily visit encounters (in-person or virtual) and documenting encounters in a timely manner.

  • Uses evidence-based male involvement curricula (24/7 Dad) to deliver education on a variety of topics from the prenatal stage to the childbirth stage and progressing to all aspects of parenting.
  • Follows up with NSB prospective and enrolled men to connect them to resources and services as needed, documents referrals and service received, and the outcome of referrals and services.

Performs general administrative duties including maintaining accurate documentation of client records, periodic assessments, statistical reports, data entry, emails and letters.

  • Participates in on-going training as needed.

Qualifications

Any combination of education and experience that would prepare the incumbent to perform the duties of the position at the appropriate level. Employees would typically have a Bachelor’s Degree.

Additional Information

All candidates must demonstrate at least one (1) year of paid, full-time employment in social work, prenatal and post-natal care, or a directly related field.

Experience in outreach and providing education to the target population.
