The Norfolk Community Services Board (NCSB) provides treatment, support, and education to the community with a focus on mental illness, substance use addiction, and intellectual/developmental disabilities. Norfolk CSB provides services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing in the community.
Today, the NCSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. The NCSB is seeking a dedicated Case Manager III to join our mobile Assertive Community Treatment (ACT) team. This role provides intensive, community-based treatment and hospital/jail diversion services for adults with serious mental illness (SMI). You’ll work collaboratively with a multidisciplinary team to deliver care in the community and in consumers’ homes.
Key Responsibilities:
- Coordinate and deliver ACT model services to adults with SMI.
- Provide direct case management, consultation, education, and interagency collaboration.
- Manage multiple priorities in a fast-paced, community-based environment.
- Work with a diverse population with co-occurring conditions.
- Participate in on-call rotation and maintain flexible availability.
Department Hiring Salary Range: $54,190 - $62,185
Requirements
Essential functions include but are not limited to:
- Develops and implements treatment plans that reflect the identified needs of the individual assessed with participation of the consumer, and his/her guardian if applicable.
- Provides linkage, monitoring, coordination, and procurement of rehabilitation counseling, residential and housing support, vocational readiness and placement support, crisis stabilization, and hospital/jail diversion services.
- Provides supportive counseling, outreach home visits, crisis intervention, counseling, and advocacy.
- Carries a caseload of 10-12 consumers, providing intensive services in the home, jobsite, and in the community.
- Provides individual, group, family rehabilitation and support counseling, independent living training, and emergency/crisis intervention when needed.
- Assists in the provision of medical services, such as delivering and monitoring medications.
- Coordinates services and service planning with other providers within NCSB and with other agencies such as, probation officers, Veterans Affairs staff, jail staff, psychiatric hospitals, counselors, landlords, vocational programs, doctors, etc., and obtains clinical documentation from other providers to ensure continuity of care.
- Where appropriate, assists with TDO's and is present at hearings for ACT Team consumers. Assists and participates in all hospital diversion, hospital in-reach, and hospital discharge and re-integration services.
- Provides case and program consultation to individuals, organizations, agencies, groups, etc. that are involved with NCSB consumers.
- Establishes and maintains inter-agency working relationships regarding the provision of PACT services and other supportive services.
- Participates in team meetings, clinical staffing and supervision.
- Maintains records related to service delivery according to applicable ethical requirements and the requirements of the NCSB and other regulatory agencies (federal, State, and others).
- Provides comprehensive assessments for adults with chronic mental illness to determine service, supports, and other needs to assist the individual to remain in the community.
- Provides emergency assessments, treatment, and referrals for team consumers, including on -call and crisis counseling services on an as needed basis.
Qualifications
Bachelor's degree in a human services field.
Two (2) years' experience in a human services field related to the areas of assignment. Some assignments may also require specific experience working with various types of client populations.
Preferred Education/Experience:Bachelor’s degree in Social Work, Psychology, Counseling, or related field (Master’s preferred).
Two (2) years of experience working with adults with serious mental illness and co-occurring disorders, preferably in a community -based program.
Additional Information
- Valid Driver's License may be required depending on assignment.
- Certification in basic first aid and CPR, depending on area of assignment.
- Certified or certifiable in TOVA preferred.
- QMHP is preferred.
Work Hours: Monday - Friday 7:00 AM - 3:30 PM; flexible hours required, including evenings, weekends (one per month), and holidays (two holidays a year).
