Job Description
This position is responsible for providing the community relationship development with health, human, and social service community partners to best encompass the community-centric needs of that position’s main site.
The position serves as a resource representative for citizens in each district area of the community to help navigate needs and issues such as food insecurity, homelessness and emergency housing, utility and rental assistance, and other targeted social needs emerging currently and in the future.
Each community has a different dynamic, and sources of need, and the navigators identify and collaborate with the community partners onsite and within the community at large through active communication, decision-making, and problem-solving.
Duties may involve participating in a support partner coordination, conducting an assessment of residents’ needs, and helping people obtain direction for the services they need.
This position will also provide the key resource and assistance necessary at the Community Resource Centers for current health partners and the community area served onsite.
