In this position, you will maintain a caseload of vulnerable, older clients who are in need of protection from harrowing and harmful circumstances.
This includes providing intensive and specialized casework and care management services for clients who need systematic support to maintain a safe and healthful independent lifestyle, as well as receiving and investigating reports of neglect and abuse of older adults and documenting all actions accurately within required timeframes.
Petitioning the court for intervention and testifying in court as needed are key aspects of this position that are further ensure the stability of our most vulnerable clients.
Your work will also involve regularly coordinating services with physicians, community agencies, financial institutions, attorneys, hospitals, nursing homes, and personal care homes, as well as responding to client emergencies.
Our team will rely on you to exercise independent judgment in achieving objectives, play close attention to critical details, and compassionately develop plans to stabilize clients in their living quarters or to arrange for a better daily living situation.
