Summary

This job involves helping older adults aged 60 and above live safely and independently by providing care management, investigating abuse or neglect, and connecting them to community resources like hospitals and courts.

Key Requirements

We are in search of a hard-working, forward-thinking Aging Care Manager 3 who is dedicated to helping assist our residents 60 years of age and older, live safe, healthy, and when possible independent lives. This is a rewarding position responsible for providing technical support to staff, providers, and subcontractors as well as investigating complaints of abuse and/or neglect. Requirements

As an Aging Care Manager 3 you will perform the following duties:

• Provide specialized care management services to Older Adults and members of their families to improve their quality of life.
• Provide protective services to older adults who are victims of abuse, neglect, exploitation, or abandonment.
• Serve as a liaison to other social service agencies, hospitals, clinics, courts, and community resources to assist clients in utilizing these resources to meet their needs.
• Consult with professional staff in other program areas to discuss assessments, diagnoses, and plans of treatment or support.
• Develop plans to stabilize clients in their current residence or to arrange for improved daily living situations.
• Testifies in court concerning various aspects of assigned cases.
• Arrange for a guardian or a representative payee to protect the Older Adults financial assets.
• Perform the full range of supervisory duties
• Perform related work as required


You may also participate in the performance of subordinates’ work consistent with operational or organizational requirements.

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.
  • Telework: You will not have the option to telework in this position.

Qualifications

Minimum Experience and Training Requirements:

  • One year as an Aging Care Manager 2 or Aging Program Assessor; or
  • One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.

  • Hiring preference for this vacancy may be given to candidates who live within Northumberland County. If no eligible candidates who live within Northumberland County apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.


Legal Requirements:

  • You must pass a background investigation.
  • This position falls under the provisions of the Older Adult Protective Services Act.
    • Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
  • This position falls under the provisions of the Child Protective Services Law.
    • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
