Full Job Description
Hennepin Health is seeking a strategic and collaborative leader to manage care coordination programs for our Special Needs BasicCare (SNBC) team. The SNBC Manager will oversee both internal and external initiatives designed to advance health equity and ensure access to essential services for members who are certified disabled. This role will also lead the development and implementation of policies and procedures for care coordination in alignment with Minnesota Department of Human Services (DHS) Medicaid requirements, partner closely with medical teams and waiver case management staff to deliver integrated care, and drive activities that promote equitable health outcomes through coordinated medical and social services.

Location and hours:
This position is hybrid and will be performed both on-site at the Health Services Building, 525 Portland Avenue South, 8th floor, Minneapolis, MN 55415, and remote as job duties require. Work hours will be Monday through Friday, with typical hours of 8 a.m. to 4:30 p.m. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin. In this position you will:- Lead and inspire a multidisciplinary team, fostering a positive, inclusive workplace culture that encourages innovation, efficiency, and improved outcomes for members.
- Set strategic direction for care coordination programs, ensuring goals and initiatives meet operational needs, comply with regulatory and contractual requirements, and align with best practices.
- Design and implement staff training programs, establish quality standards, define performance metrics, and provide ongoing supervision and evaluation to support professional growth.
- Develop and enforce care coordination policies and procedures in compliance with DHS contracts, county policies, and state and federal regulations.
- Coordinate member-facing activities, building collaborative processes across Hennepin Health, county departments, provider partners, and external organizations to deliver tailored solutions for SNBC members.
- Manage financial planning for care coordination, including budget development, expenditure tracking, and advocating for resources to support member benefits, innovative programs, and operational needs.
- Advance department strategies that promote equity and inclusion, aligning with county goals to reduce disparities and create a welcoming workplace.
Need to have:- One of the following:
- Nine years of experience related to organizational operations or programs or a specialty area appropriate to the position.
- Bachelor's degree in business administration or other field appropriate to the position and five years of experience related to organizational operations or programs or a specialty area appropriate to the position.
Nice to have:- Experience:
- Supervising, developing, and directing the work of staff at the individual contributor level who are managing large caseloads within a team.
- Supporting populations who are certified disabled or present with complex medical needs and social vulnerabilities.
- Designing processes to implement order, predictability, and consistency.
- Delivering care coordination or case management services.
- Leading disparity reduction work from a healthcare perspective or workplace inclusion initiatives.
- Using MnChoices, Snowflake, electronic health records, Microsoft Teams, Excel, and other related computer applications.
- Working with a managed health care plan.
- Knowledge of:
- Principles and practices of care coordination; service referral within a managed healthcare plan, county system, and community-based agencies.
- Management of information systems relevant to care management, coordination, and reporting.
- Performance measurement and program evaluation including those related to disparities in healthcare.
- Budget evaluation, preparation, and monitoring.
- Ability to:
- Serve as a trusted representative of Hennepin Health, building strong relationships within Hennepin County and with external stakeholders, including state agencies, health systems, managed care organizations, community-based organizations, and other partners.
- Create clarity in complex or ambiguous environments.
- Communicate effectively and professionally, delivering clear, concise, and well-structured messages in both verbal and written formats to diverse audiences.
- Analyze and interpret complex technical data, using insights to inform strategic planning, monitor program performance, and support evidence-based decision-making.
