DOC is seeking a highly motivated and qualified Registered Nurse 3, Infection Prevention for the Clallam Bay Correctional Center located in Clallam Bay, Washington.The mission of Department of Corrections (DOC) is to improve public safety. The mission of Health Services is to provide necessary comprehensive, quality health services and to promote wellness in preparation for return to the community. The mission of the Infection Prevention Nurse is to prevent harm caused by infection to patients and staff and ensure quality health care for patients with transmissible infections which in turn improves public safety.The Infection Prevention Nurse/Registered Nurse 3 directly oversees the facility Infection Control and Prevention Program. The Infection Prevention Nurse helps to develop, maintain, and evaluate this program, by supporting, educating and directing staff.
Requirements
Assist in the care and management of patients with communicable diseases.
Provide clinical oversight of HIV-infected patients, including, but not limited to, medication
adherence counseling, facilitation of care and release planning.
Manage the Hepatitis C Program at the facility, including, but not limited to, managing databases,
implementing Hepatitis C protocols, providing support to practitioners regarding Hepatitis C care,
participating in Hepatitis C related conference calls, and offering testing for Hepatitis C as
appropriate.
Manage Tuberculosis (TB) screening and management according to DOC Policies and protocols,
including monitoring adherence to treatment, laboratory testing, acting as a liaison to public health,
tracking patient data, training staff and overseeing the implementation of screening.
During outbreaks or for certain infectious diagnoses, monitor and collect specific data as directedregarding the health status, clinical condition, or situation of patients, families, groups, and/orcommunities by interview, observation, inspection, and examination.
Report the results of the datacollection to Health Services practitioner(s), as clinically indicated.
Carry out the plan of care by performing common therapeutic interventions, informing others aboutthe urgency, time and methods to deliver health care on and off site.
Determine when needs for care or services are beyond the Registered Nurse scope of credentialing and training, require clarification, require another type, or level of intervention.
Take action as necessary to obtaindirection from the Health Services practitioner(s) and accomplish prescribed interventions in a timely and clinically responsive manner.
In conjunction with Health Services practitioner(s), coordinate health care received by patients by completing encounter forms for documentation and scheduling of needed services.
Complete incident reports as directed.
Respond to verbal and written requests from patients, as directed.
Effectively communicate verbally, in writing, and via computer or other electronic media with patients, their families, other health care personnel, custody, and other institutional personnel.
Legibly document subjective and objective clinical findings, the rationale, and the actions taken to deliver health care in the health record, computerized database, and on other DOC approved forms.
All documentation will be in accordance with the Health Records Policy [DOC 640.020] and associated protocols.
Protect the confidentiality of protected health information (PHI) and the health record as mandated by law, professional standards, DOC Policy Directives, facility Operational Memoranda, and Health Care Unit procedures.
Treat any and all health records or health information as confidential and privileged.
Provide information or advice consistent with statutes and procedures on health care record
confidentiality.
Maintain accurate, legible, and complete records of patient care, including chart documentation using appropriate format, of all patient requests for medical attention, objective data collected, treatment administered, evaluation of treatment effectiveness and patient education provided.
Assist in writing nursing care plans and interdisciplinary treatment plans and complete other entries to support the problem-oriented health care record.
Establish therapeutic relationships that preserve professional boundaries with patients to counsel, teach, and assist patients to reduce health risks, improve their ability to provide self-care, manage symptoms and side effects, to participate in prescribed treatment regimen, and to make informed decisions about health care and treatment.
Use tools, techniques, physical and cognitive skills, and abilities to practice nursing safely and effectively in the correctional setting and within the scope of practice defined by state law and administrative code so that patient care provided is safe, effective, efficient, and clinically
appropriate.
Assess, monitor, and interpret objective and subjective data collected about the health status, clinical condition or situation by interview, observation, inspection, and examination to identify patterns and variances from the desired outcome.
Monitor and enforce Infection Prevention & Control according to DOC Policy 670.000, DOH Health,
Environmental, & Safety Standards, DOC Policy 590.100, DOC Policy 890.600 and all infection
prevention protocols and guidelines.
Ensure that patients who have communicable diseases or whose health is at risk as a result of
communicable diseases are assessed, evaluated and managed according to standardized infection
control guidelines.- Participate in facility Infection Control Meetings.
- Participate in facility audits with Safety Team.
- Participate in statewide bi-weekly Infection Prevention Nurse (IPN) Meetings.
- Attend quarterly statewide IPN meetings/learning sessions.
- Report communicable diseases to the Department of Health (DOH) and/or local health jurisdiction
as required by law. - Monitor and enforce all infectious disease protocols, (ex. influenza, scabies, lice, norovirus, postexposure
prophylaxis, etc.). - Collaborate with Occupational Nurse Consultants (ONC) after staff exposures to blood and/or body fluids, offering assistance with source person testing and post exposure management.
- Collaborate with public health in the event of an outbreak and implement control measures as appropriate.
- Provide sexually transmitted infection (STI) education, data management and reporting.
- Provide notification and education prior to approval for extended family visits as appropriate.
- Track and manage data for infection control including analysis and reporting of trends over time via the Surveillance Log.
- Manage the patient vaccination program, ensuring vaccines are administered and documented appropriately.
- Work with practitioners to teach, educate, monitor and enforce compliance with proper isolation of patients with transmissible diseases.
- Help monitor and enforce handwashing and the proper use of personal protective equipment.
- Help monitor and enforce proper cleaning and disinfection of rooms used for patients in Health Services, including terminal room cleaning.
Oversee the practice of all CMAs, CNAs, LPN2s, LPN4s, and RN2s under this position’s
supervision.- Teach, instruct, monitor, supervise and evaluate the implementation of tasks and the results of patient care assigned to staff nursing personnel.
- Ensure that nursing personnel are in attendance, capable and technically competent to practice nursing as assigned.
- Hold staff accountable for reporting and performing work as scheduled so that staff safety and patient care are not compromised.
- Assign work to be performed by other nursing personnel, based on the scope of practice allowed by law for the staff assigned the work, and their knowledge, skills, credentials and abilities to perform the work.
- Follow the guidelines of the Collective Bargaining Agreement (CBA) to ensure processes and staff management are fair and consistent.
- Perform staff evaluations in a fair, consistent and timely manner to share positive feedback, and/or constructive critiques to encourage, challenge and engage staff. Highlight opportunities for growth, professional development, and areas of expertise and success.
Collaborate with all agency divisions to support infection control and prevention.- Monitor and enforce sanitation standards for cleaning of living units (showers, kitchens, cells, etc.).
- Monitor and enforce sanitation standards for the cleaning, sanitizing and/or sterilization of medical and dental equipment and supplies.
- Monitor and enforce sanitation standards for the cleaning, sanitizing and/or sterilization of barbering tools, equipment and supplies.
- Act as consultant to Safety, Custodial, and Janitorial Services.
- Act as consultant to Laundry Services.
- Act as consultant to Food Services Program.
- Develop and monitor policy for standardization of cleaning products including the use of, and necessity for, bleach.
- Work with Occupational Nurse Consultants to support the vaccination and health of staff.
- Evaluate, monitor, and order medical supplies to ensure necessary and sufficient quantities are available for infection control purposes, coordinating with office and warehouse staff as needed.
Assume accountability and responsibility for quality and efficiency of patient care services and delivery.
- Plan the delivery of health care, defining timeframes and pathways for delivery of services, prioritizing and sequencing care to be provided, ensuring continuity of care over time and place.
- Use critical thinking to determine when a patient's condition dictates the need for a higher level of intervention or different modality of care.
- Identify when staff require clarification of orders to provide practitioner-prescribed interventions in a timely and clinically responsive manner.
- Analyze and evaluate the structure, process, and efficiency of service delivery, health care record documentation, and other nursing procedures to prevent complications, improve patient outcomes, and increase the quality of care.
- Review medication administration and the area(s) where dispensed medications are stored so that medication is administered to patients in the infection control and prevention program safely and effectively.
- Manage the human, medical and office supplies, and resources of the clinic in a manner that is appropriate and financially responsible.
- Provide accurate, authoritative information to groups or communities, including colleagues and correctional personnel so that good clinical practices and healthy behaviors are promoted and health risks are reduced.
- Communicate effectively verbally, in writing, and via computer or other electronic media with patients, their families, other health care personnel, custody and other institutional personnel, so that information is solicited to identify problems and develop a solution.
- Ensure that plans to deliver care are implemented by self and others are revised as necessary, resulting in safe, timely, and clinically appropriate service provision.
Ensure staff participation in training and orientation.- Train, monitor, and supervise staff to ensure that staff respond to outbreaks and other infection control emergencies involving patients, staff and/or visitors in an appropriate, timely and clinically competent manner, and emergency equipment and supplies are maintained in good working
order, within expiration/usage dates and organized for immediate access and use. - Participate as requested in planning the emergency response for natural disasters, drills and other emergencies on all campuses of the institution.
- Practice and review the medical components of the emergency response plan as requested and update as needed, at least on an annual basis.
- Facilitate the orientation process for new nursing staff and agency nurses regarding the Infection Control and Prevention Program.
- Provide technical guidance in using computer applications, (ex. CIPS, OMNI) and other equipment and software programs.
- Assist in updating Infection Prevention training manuals and calendars at least annually.
Actively participate in clinic/facility-wide Coordinated Quality Improvement Program (CQIP) projects
and programs.- Attend facility CQIP meetings as appropriate and report back to nursing and support staff to keep nursing staff engaged in improving patient care.
- Serve as the lead for at least one local quality improvement project per year and engage nursing staff in the process to ensure all employees have an understanding of the principles of quality improvement and the goals for the organization.
- Participate/represent facility on statewide quality improvement initiatives; share our processes and learn from other facilities; implement best practices in patient care.
- Participate in Health Services Quality Improvement activities that impact the Infection Control and Prevention Program.
- Actively participate in multi-disciplinary, facility, statewide, and management team meetings.
- Assist in the development and implementation of policies and procedures for health services.
- Practice in accordance with DOC Policy and the Offender Health Plan.
- Participate in formal clinical oversight according to facility and DOC Health Services Clinical Oversight Structure.
Self-evaluate and practice professional conduct at all times.- Evaluate one’s own practice for technical competency and safety, obtain education or training to retain or improve skills and abilities based upon the work assigned, and inform the immediate supervisor if needed.
- Be in attendance, capable, and competent to practice as assigned. Report for and perform work as scheduled so that staff safety and patient care is not compromised.
- Using established methods, brief appropriate personnel on pertinent information, before departing from duty, to ensure continuity of patient care.
- Work overtime and callback shift as necessary to maintain service delivery, safety, and security.
Qualifications
Required Qualifications:
Initial and continuous unencumbered licensure in the State of Washington as a Registered Nurse
OR
An unencumbered Multi-State License from a participating Compact Licensure State and completion of the MLS
demographic survey.
AND
Two years’ experience working as a Registered Nurse.
Training by the American Heart Association in Healthcare Providers CPR.
Proficient using computer-based programs to schedule, monitor and track patient information, perform data entry
and word processing, and communicate via email, as well as other programs that may be needed to complete
assigned work.
Ability to work independently and with limited supervision.
Certification as an Infection Prevention Specialist or at least 2 years clinical experience in HIV/Viral Hepatitis/sexual
transmitted infection/TB program management, infection prevention, and/or outbreak investigation.
Ability to research, accumulate, organize, and communicate technical information both orally, in writing, in person,
and using technologies such as telephone, electronic mail, and computer equipment.
Demonstrable skill in data collection, data management using Excel or Access, data analyzing and presentation of
data to groups and individuals.

Bachelor's degree or higher.
Experience working as a Registered Nurse in a correctional facility or public health.
Certification as a Correctional Health Professional with NCCHC (National Commission on Correctional Healthcare).
Experience in patient case management
