Full Job Description

The Population Health Bureau Health Access Division is hiring a Program Coordinator to serve as the primary resource for uninsured Davidson County residents seeking primary health care, specialty care, and pharmacy services through Project Access Nashville, Project Access Nashville Specialty Care, and the Affordable Care Act (ACA) Marketplace. The Program Coordinator works with a team of individuals who support insurance enrollment and resource navigation for uninsured and underinsured residents. This is a non-supervisory position that reports to the Health Access Division Director.

Requirements

1.

Using a high degree of subject matter expertise, excellent customer service skills and knowledge of current enrollment requirements during face-to-face encounters, assists residents to complete enrollment in an available insurance plan: Prenatal Presumptive for pregnant women and the ACA Marketplace (during staff shortages); Maintains all required certifications and applicable trainings for Health Access Division operations including but not limited to Center for Medicaid and Medicare Services (CMS), TennCare and Tennessee Eligibility Determination System, Patient Tracking Billing Management Information System (PTBMIS) and Patagonia Electronic Health Record; Completes data entry and maintains accurate customer records in accordance with Health Access Division policies and protocols.

Assists with eligible uninsurable residents’ navigation of Project Access Nashville resources, including Project Access Specialty Care as well as Safety Net resources; uses problem-solving and systems thinking skills to support resident connection to a medical home; provides education to and referrals for residents on local community, faith-based, and Federally Qualified Health Centers (FQHC) resources.

3. Serves as the Health Access Division lead coordinator for outreach and education events; promotes Health Access services in community settings and partners with internal and external work units/agencies at least quarterly to raise awareness of insurance and healthcare resources.

4. Collects data on Health Access Division-related performance metrics; identifies service delivery gaps and recommends solutions to enhance program effectiveness; prepares quarterly and annual performance reports as well as ad hoc reports as requested.

5. Performs other duties as assigned, consistent with position classification, even if not related to job title; may support program coordinator team and oversee health access initiatives, ensuring alignment with organizational goals and compliance with state and federal regulations.

Qualifications

Bachelor’s degree from an accredited college or university in Public Health, Health Education, Community Health, or a related field, and two (2) years of progressively responsible full-time paid employment in a related field are required.

Additional Information

· Must be available to work occasional weekend and evening hours to support outreach and community engagement activities as well as ACA Marketplace open enrollment (November 1 – January 31st annually) and special enrollment periods, interfacing with residents throughout Davidson County.

· Must demonstrate data entry accuracy with minimal key stroke errors.
