This role focuses on helping people experiencing chronic homelessness in King County by managing referrals to affordable housing units and building strong connections with local communities.
In the first nine months, you'll work with partners to improve how people are selected for housing in a fair and inclusive way, especially to reduce racial disparities.
After that, you'll engage with neighborhoods around housing sites to ensure smooth operations and support quality checks on housing providers—ideal for someone passionate about social equity and community building.

Health Through Housing invests in the premise that a person's ability to be housed is a fundamental component of that person's ability to gain and maintain health.The Paramount Goal of the HTH initiative is the creation and operation of 1,600 units of affordable housing with supportive services for eligible adult households in King County that are experiencing chronic homelessness or that are at risk of experiencing chronic homelessness.
Information on the current scope and status of the initiative is available on the HTH Data Dashboard.In addition to the initiative's Paramount Goal, Health through Housing has seven ordinance-adopted supporting goals, the first of which is to annually reduce racial-ethnic disproportionality among persons experiencing chronic homelessness in King County.
HTH is staffed within the Housing and Community Development (HCD) Division of King County's Department of Community and Human Services.
The mission of HCD is to increase housing stability and develop strong communities throughout the King County region.
We strive to be an anti-racist division and collaborate with our partners to center historically excluded and systemically marginalized people.We are seeking a Health through Housing (HTH) Referral & Community Coordinator, Project/Program Manager II, to join an initiative developed to create and operate 1,600 units of affordable, supportive housing for eligible adults experiencing chronic homelessness or at risk in King County.
For the first 9 months, you will lead jurisdictional stakeholders and housing operators in administering and continuously improving referral operations to identify and house formerly homeless individuals in a person-centered, transparent way that advances HTH’s top goal of reducing racial-ethnic disproportionality among people experiencing chronic homelessness.After that initial period, you will transition to serving as the primary liaison between PSH sites and the surrounding community, playing a hands-on role in community engagement, implementing good-neighbor processes, and conducting interest-holder mapping, outreach, and mobilization.
You will also be a member of the Health through Housing Quality Management Team, led by the Quality Management Coordinator, and supporting the team with compliance audits of HTH housing operators.This recruitment is to fill a Term-Limited Temporary (TLT) position that is anticipated to be two years in duration but is subject to change dependent on business needs.
If filled by a current King County employee who has passed their initial probation, the TLT will be as a Special Duty Assignment (SDA).
Requirements To be considered minimally qualified applicant must demonstrate a background that provides the knowledge, skills and ability to:Ensure housing operators' compliance with operating procedures and HTH Implementation Plan requirements related to community engagement and local and regional referral.
Proactively identify areas for continual process improvement and, when necessary, lead those improvements.Independently work to identify potential community-based partners (such as non-profits and human service agencies) in each HTH jurisdiction and build operational partnerships with those interest-holders, sometimes involving formal agreements.Independently work to identify and build relationships with community-based champions of the HTH model within each HTH jurisdiction.
Organize and mobilize champions for involvement in good neighbor processes and other community engagement opportunities Screen incoming referrals for HTH eligibility for some HTH properties and occasionally facilitate case conferencing meetings in order to fill designated local referral units, surface and troubleshoot process issues, and ensure alignment with racial equity goals of the HTH initiative.Act as subject matter expert and primary contact for any public or partner inquiries related to HTH referral and community engagement.
Develop and monitor Microsoft forms, a referral-specific email inbox, and other public-facing tools as needed.In coordination with Performance, Measurement and Evaluation (PME) staff, identify and track key performance indicators such as referral volume, source and demographics, vacancy rates, referral disposition, and other operational data points.Support the Compliance auditing process by evaluating operators based on various quality metrics.
Build strong working relationships and maintain open communication with key staff at King County Regional Homelessness Authority (KCRHA) and host city governments and serve as the primary point of coordination between the Health Through Housing initiative and Coordinated Entry.
Qualifications At least three years of previous professional and/or volunteer experience serving vulnerable populations, examples of which include survivors of gender-based violence, immigrants and refugees, persons with disabilities, persons living in poverty, persons disproportionately impacted by the criminal legal system, and people experiencing homelessness- within all of which Black, Indigenous and People of Color (BIPOC) communities are overrepresented.Demonstrated understanding of equity and social justice concepts and the effects of place, race and systems-based inequities in relation to homelessness in King County.
Ability to apply an equity and social justice approach to all areas of the work including interpersonal communication, self-identity, and team dynamics.Self-starter with experience in highly collaborative, cross-functional teams or in similar working environments.Ability to work independently, identify goals, develop strategies to achieve them, and drive for results with minimal guidance.Knowledge and strong belief in "Housing First" philosophy and strategies.Ability to effectively facilitate meetings and generate discussion among diverse stakeholder groups, driving toward consensus.Critical thinker and problem solver with the ability to anticipate challenges and proactively integrate those insights into program design, and implementation, and continuous improvement.Demonstrated ability to handle conflict and difficult interactions with discretion and diplomacy.Ability to read, retain and recall details and functional knowledge of lengthy procedural manuals such as HTH Operator Handbook and the HTH Implementation Plan.Working knowledge of community-based programs and resources available in King County within the realm of health and human services.Excellent written and oral communication skills that can be applied to tasks including correspondence, development of standard operating procedures, and public speaking.Ability to meet work deadlines, stay on task with frequent interruptions and balance sometimes competing priorities.Must have the ability to attend frequent meetings at various locations throughout King County in a timely fashion.DESIRED EXPERIENCE, KNOWLEDGE AND SKILLSFamiliarity with the homeless services and housing landscape in King County, including working knowledge of King County's Coordinated Entry (CE) process and Homeless Management Information System (HMIS).Experience with homeless outreach and/or supportive housing at the direct service level.Personal or professional experience with community organizing, advocacy, or relations, particularly around contentious or high-profile issues.ADDITIONAL POSITION INFORMATION:

The HCD team works in a hybrid model, with days in the office as well as telecommuting. The ratio of remote to onsite work will be dependent on business needs and is subject to change.
The primary onsite location is Chinook, 401 5th Ave, Seattle, WA 98104.
Employees must reside in Washington state and within a reasonable distance to their King County worksite to respond to workplace reporting requirements.King County has a robust collection of tools and resources to support working remotely.
The individual selected for this opportunity will join an innovative and progressive team that is redefining how we work as we transition to the department's hybrid environment.
Employees will be provided with a County-issued laptop and must maintain a home workspace with an internet connection where they can reliably perform work and remain available and responsive during scheduled work hours.
