DSHS Lakeland Village Registered Nurse 3 - Case Manager


We're looking to hire a Case Manager Registered Nurse 3 at Lakeland Village in Medical Lake, Washington.

Opportunities performed on-site at this work location at least three days a week may be eligible for an additional 5% premium pay in addition to the advertised salary.

Some of what you'll do:
  • Coordinate nursing care for an assigned caseload of residents with a diagnosis of intellectual disability and/or psychological disability, who may exhibit challenging behaviors and other physical health concerns
  • MDS (Minimum Data Set) Coordinator: Develops, reviews, and maintains resident’s Care Plan to promote physical and psychological health. Enters information into computerized MDS/Care Plan software.
  • Ensures compliance with state and federal guidelines regarding Care Area Assessment (CAA) and MDS summaries.
  • Provide assessment, nursing diagnosis, planning, implementation, and evaluation
  • Act as a nursing liaison to the interdisciplinary team (IDT), the RN supervisor/designee on client health issues as a designated Case Management Resource Nurse
  • Responds to client change in condition, works with the provider, and helps track medication reviews or planned changes with the provider
