The primary responsibility is to serve children and adults who have been diagnosed with serious and persistent mental illness and who are in need of assistance from other community services.
An important aspect is to provide intensive case management as a primary mode of service which will assist the consumer in gaining access to resources such as medical, housing, social, educational, vocational, and other services.
The intensive case management services must be goal directed to reduce avoidable inpatient hospitalizations and enhance quality of community living and independent social functioning.
