Correction Specialist 3

Tumwater - Full Time/Perm

Salary Range: $6,750 –$8,750 Monthly

The Department of Corrections is seeking a highly motivated and qualified individual to fill a permanent position of the Corrections Specialist 3 (CS3).

Working for the Department of Corrections, Health Services Division, in collaboration with community partners, this position will contribute to staff and community safety by delivering individualized case management and continuity of care for behavioral and medical services to eligible incarcerated individuals per State Opioid Response Grant (SOR) contract.

Under the supervision of the SOR Grant Corrections Specialist 4 Program Manager, this position will provide internal and external collaborative reentry planning, educational and transitional services for program participants.

Continuity of care will start in prison and/or reentry centers and continue into the community post-release.

This position will provide critical services that meet the objectives and deliverables of the SOR grant.

All of the activities of the Corrections Specialist 3 (Medication for Addiction Treatment Coordinator, MATCo) will directly support the SOR Grant program with coordinated linkages to support successful reentry.

This position supports the mission of DOC, Health Services Division by offering linkages and community support to incarcerated individuals to help decrease recidivism, reduce relapse/overdose risk and promote ongoing health and wellness during transition from incarceration.

As a part of the Heath Services Reentry Team, this team is part of the mission to provide comprehensive, specialized healthcare discharge planning and community linkage to individuals transitioning from incarceration to support integration and wellness.

Requirements

Provides continuity of care planning from prison to community for a specialized SOR Grant eligible caseload.

  • Working with program participants, Health Services staff, community stakeholders, applicable program protocols and resources to complete a SOR Grant transition plan that includes:
  • Identify community resource needs in partnership with facility Health Services Reentry team, including Substance Use Treatment (opioid, methamphetamine), health and mental health.
  • Identify participants’ strengths that support healthy recovery from substance use disorder.
  • Coordination of providers and treatment into the community (may include conference calls or visits with individuals and community providers) to include appointment for Medications for Addiction Treatment scheduled post release.
  • Provide 30 days post release case management support. This includes Recovery Support Services (RSS) which means non-clinical services such as food and transportation that assist individuals and families working towards recovery from substance use disorders (SAMSHA definition).
  • Collaborate with facility Addiction Medicine team (i.e. MOUD RN, Addiction Physician.) and Health Services Reentry Teams (i.e. Social Workers, Reentry Nurses,
  • Provides case management to include screening, education, and referrals.
  • Maintain communication with program participants, DOC staff, and service providers to ensure continuity of care between DOC and community-based providers.
  • Coordinate with facility Health Services for identification of those receiving MOUD prior to release.
  • Services will be offered at least once in person to program participants (best effort based on facility constraints).
  • Work with SOR program team members, corrections staff, facility healthcare staff and community providers to meet with the program participant to identify resources and supports in the participant’s county of release.
  • Educate clients one on one and in group settings on medications for addiction treatment and process of being prescribed MOUD prior to release.
  • Respond to kites, kiosk and/or other messages from incarcerated individuals professionally and as directed by Manager.
  • Establish and maintain, for the benefit of the program participants, frequent communication with systems and community-based resource providers to ensure current knowledge of capacity, eligibility criteria and types of service.
  • Engage in post-transition assistance with program participants as outlined in SOR grant following release from incarceration.

    Activities may include: Maintain telephone communication with program participants, reviewing and discussing strategies for compliance, ongoing provision of information related to available services and resources, telephone assistance and advocacy in the process of accessing and/or maintaining services and resources.

Maintain comprehensive, current program participant records

  • Collect, enter, and track all grant-required program performance measures and data.
  • Report grant-required program performance measures and data to supervisor as requested.
  • Enter data per DOC HS policies and protocols in electronic systems to include OMNI HS Reentry encounters.
  • Complete assessment tools as required by grant (such as GPRA or SUPRT).

Collaborate with internal and external partners

  • Participate in work groups, coalitions, committees or other types of meetings relevant to the SOR II grant.
  • Build partnerships with and ensure continuous engagement of a wide range of systems and community-based agencies in an effort to leverage all available resources that support a healthy recovery.
  • Collaborate with community MOUD providers to develop relationships and to facilitate Warm Hand-Offs for Individuals being released from incarceration or transferred to partial confinement.
  • Identify problems and potential solutions for access to care that arise during transition from prison healthcare to community healthcare.

Other duties as assigned

  • Participate in training and professional development opportunities as required by the Agency and the SOR program.
  • Engage in other HS related quality assurance or special projects related to the SOR Grant and behavioral health.
  • Other Duties as Assigned.

Qualifications

Required qualifications:

  • 4 years of professional experience providing healthcare case management services in adult or juvenile corrections, social services, or closely related field, 2 years working with Medicaid systems.
  • Proficiency using Microsoft Office software (Word, Excel, Outlook). Excellent written and verbal communication skills

Preferred/Desired Qualifications:

  • Bachelor's Degree from an accredited college whose accreditation is recognized by the US Department of Education or the Council for Higher Education Accreditation (CHEA), or a foreign equivalent.
  • Four years of professional experience in adult or juvenile corrections, social services or closely related field.
  • Experience working in correctional environment.
  • Substance Use Disorder Professional License
  • Ability to work independently and within a team while exercising good judgement and promoting positive working relationships with incarcerated individuals, staff, and partners.
  • Ability to build strong rapport with justice involved individuals
  • Demonstrate a willingness to receive, internalize, and learn from feedback to exhibit professional growth.
  • Knowledge of prison-, jail- and community corrections-based standards and rules with the ability to analyze information to identify problems and develop subsequent action and strategic plans.

NOTE: Additional qualifying experience will substitute for up to two years of the desired education. A Master’s degree will substitute for one year of experience.
