Correction Specialist 3
Tumwater - Full Time/Perm
Salary Range: $6,512 –$8,750 Monthly
The Department of Corrections is seeking a highly motivated and qualified individual to fill a permanent position of Corrections Specialist 3 (CS3).
The Corrections Specialist 3 (CS3) will provide reentry support and resources for those releasing as a result of the Blake decision and other sentencing reform operations. This position will supervise continuity of care team members providing continuity of care reports to managed care organizations and fee for services agencies. Requires collaboration with internal and external partners to include Health Care Authority, providing critical liaison role for patient healthcare needs post release.
The CS3 is primary liaison between Health Services Reentry, Reentry Centers, Community Corrections, and Reentry Navigators to facilitate continuity of care to individuals during their transition from prison to community to ensure linkage.
The CS3 position supports the Department’s mission of improving public safety by positively changing lives by providing support related to healthcare needs for those releasing to the community to ensure continuity of care.
This is a critical role in the Health Services Reentry Team mission to provide comprehensive, specialized healthcare discharge planning and community linkage to individuals releasing from incarceration/ transitioning to partial confinement to support integration and wellness.

Requirements
Oversight, Coordination and Implementation of Continuity of Care Program
- Administrative supervision of Continuity of Care specialists to include expectations and evaluations.
- Creating protocols and processes for Continuity of Care Program.
- Primary point of contact for Managed Care Organizations (MCO) to include facilitation of information sharing for releasing individuals’ healthcare needs continuity of care.
- Build and maintain partnerships with MCOs and ensure Continuity of Care team provides data summaries to MCOs for all releasing individuals with an MCO.
- Identify processes with Health Services Reentry Leadership Team for sharing other forms/ data with MCOs and other DOC partners to facilitate continuity of care.
- Responsible for tracking data related to scope of work.
- Identify and Update Health Services reentry resources including Sharepoint.
- Collaborate with Reentry Division to include information about available resources
- Facilitate healthcare screening for partial confinement transfers in partnership with facility Health Services Reentry Teams.
- Support Health Services Reentry Teams with quality assurance of release planning packets, resources provided by direct reports.
- Participate in Health Services Reentry Leadership Meetings, and other Health Services meetings relevant to continuity of care.
Case Management to facilitate continuity of care with a focus on troubleshooting access to care issues post-release.
- Coordinate with Prisons, Reentry and CCD as needed to provide health care related resources and information and to solve emergent issues.
- Respond to and develop solutions for problems that arise during transition from prison to community in navigating/ accessing healthcare and health related social needs.
- Medication coordination between Prisons, Reentry, CCD, Health Services for situations where individuals don’t have needed prescriptions upon community transition
- Partner with Health Care Authority to develop and implement process flows to support releasing population healthcare needs to include AI/AN individuals and veterans.
- Attend Reentry Team Meetings and/or Multidisciplinary Team Meetings for individuals reentering the community and tracking their healthcare needs
- Manage specialized caseload of resentencing cases, supporting transition of healthcare needs pre-release to post-release in partnership with Health Services Reentry Teams.
- Collaborate with all DOC navigators for individuals transitioning back to the community including resentencing cases, immediate releases, complex healthcare needs, other healthcare related support.
- Identify quality assurance measures to implement and track related to releases associated with sentence reform actions.
- Support Extraordinary Medical Placement with tracking and processing referrals, and other tasks at the direction of EMP Coordinator.
- Communicate with social work staff and other health services reentry programs to assist with continuity of care for those with high health services needs reentering the community.
- Provide support after release to individuals in the community who have health care questions and need assistance to include release medications and health insurance navigation.
- Track caseload on spreadsheets and maintain databases related to work.
- Support new processes related to streamlining work and partnering between divisions related to continuity of care work.
Other Health Services reentry related tasks and duties as assigned
- Maintain standards of confidentiality
- Cover for Health Services Reentry programs as needed
- Continuous Quality Improvement projects to include participation in mapping and other efforts.
Qualifications
Required qualifications:
- 4 years of professional experience providing healthcare case management services in adult or juvenile corrections, social services, or closely related field.
- 2 years working with Medicaid systems.
- Supervisory Experience.
- Proficiency using Microsoft Office software (Word, Excel, Outlook). Excellent written and verbal communication skills.
- Note: Additional qualifying experience will substitute, year for year, for up to two years of the desired education.
Preferred/Desired Qualifications:
- Bachelor’s Degree in social services related field from an accredited college or university whose accreditation is recognized by the US Department of Education or the Council for Higher Education Accreditation (CHEA).
- Working knowledge of Managed Care Organizations and healthcare coordination.
- 2 years supervisory experience.
- Proficiency using Microsoft Office software (Word, Excel, Outlook). Excellent written and verbal communication skills.
- Ability to communicate clearly and concisely, both orally and in writing, with DOC-involved individuals, staff at all levels within the Agency and with a broad range of system- and community-based external stakeholder organizations to include other state agencies.
- Ability to build strong rapport with justice involved individuals.
- Ability to work both independently and in close collaboration with system- and community-based stakeholders.
- Demonstrate a willingness to receive, internalize, and learn from feedback to exhibit professional growth.
- Knowledge of prison-, jail- and community corrections-based standards and rules with the ability to analyze information to identify problems and develop subsequent action and strategic plans.

- Must be able to work in spaces, climb stairs, walk long distances, and frequently lift up to 30 pounds. Under rare circumstances this position may be required to lift or carry up to 50 pounds and push up to 100 pounds.
