Under the general supervision of the Regional Children and Youth with Special Health Care Needs Nurse Unit Manager, (CYSHCN) provides nursing care, client education and counseling, and coordination of services for clients and their families. Serves as a medical resource for the interdisciplinary CYSHCN team. Screens clients for eligibility for CYSHCN and other healthcare services and programs. Provides Public Health nursing services in a culturally competent and professional manner.

Utilizing the nursing process, assesses the health of clients and assists in developing, implementing, and revising care plans to include appropriate testing, interventions, education, and counseling based on identified needs. Prioritizes identified client/family needs for those seeking or receiving CYSHCN services. Provides nursing care and participates in providing patient education, anticipatory guidance, and care coordination for clients /families within the CYSHCN population. Determines eligibility for financial assistance according to the CYSHCN policy manual and authorizes services for eligible clients as needed / requested. Adheres to all programmatic and practice policies, including those in the Nursing Professional Practice Manual, Health Services Policy Manual, Health Records Policy Manual, and the Standing Orders Manual. Assists with screening and physical assessment of campers on check-in days at Camp Burnt Gin as assigned by supervisor. Administers intramuscular, subcutaneous, and intradermal injections and oral medications following best practices when needed. Documents care/services per agency policies and procedures.

Serves as care coordinator for assigned caseload and implements plan of care via visits or contacts with client/family. Functions as a member of an interdisciplinary team and coordinates case findings and services with other disciplines as needed. Makes referrals for medical, socioeconomic, mental, nutritional, and educational needs as identified and appropriate. Provides nursing consultation when requested by other CYSHCN disciplines. Maintains knowledge of community resources relevant to the CYSHCN population.

Collects and reports on characteristics of assigned caseload, services provided, and outcomes as requested by supervisor. Participates in program and discipline audits as requested. Participates in the development and review of CYSHCN policies and procedures related to the delivery of CYSHCN services as requested. Participates in staff meetings and shares relevant information with the regional team. Assists with the training and mentoring of new CYSHCN staff.

Completes CYSHCN program and agency required training per policy. Maintains nursing licensure and credentialing per agency policies (30 contact hours every two years). Maintains current certifications and competencies in First Aid, CPR, AED, and injection administration with 100% compliance. Completes annual required training and drills related to emergency preparedness and response annually. Represents program or region on committees, work groups, and at community events as assigned. Identifies training needs to supervisor. Participates in Medical Equipment Power Shelter (MEPS) operations as assigned during emergency response events and responds promptly and effectively to client emergencies during public health emergencies and outbreak response efforts. Maintains flexibility, professionalism, and completes other duties as assigned.



Requirements

State Minimum Requirements: Licensure as a registered nurse by the State Board of Nursing for South Carolina. Graduation from an accredited school of nursing.


Institutions of Higher Learning must be recognized by the Council for Higher Education Accreditation.

Agency Additional Requirements: Associate of Science degree in Nursing from an accredited school of nursing. Minimum of at least one (1) year experience as a Registered Nurse.

Qualifications

Bachelor of Science degree in Nursing. Experience working with Children and Youth with Special Health Care Needs (CYSHCN) or their families, children’s health services, maternal and child health, public health, or pediatrics.

DRIVING RECORD: If this position requires the applicant to possess a valid driver’s license to operate a state vehicle or personal vehicle, any applicant being considered in the final stages of selection for the position will be required to provide a certified copy of a 10-year driving record.
