Early Childhood Care Coordinator

Requirements

Responsibilities:
  • Provide care coordination services for youth from infancy through age eight across the state
  • Serve as an intermediary to connect families with appropriate needed services and support as applicable; answer related questions; facilitate application processes; and direct families to appropriate public agencies.
  • Research and investigate learning, enrichment, and other opportunities for families and patients.
  • Consult with other care coordinators on the team, both the Preschool Development Grant and Integrated Services Program, to discuss best practices for service delivery and share findings with the team.
  • Contribute to team's Continuous Quality Improvement (CQI) process and Results Based Accountability (RBA) outcomes.
  • Serve on various committees, work groups, and planning teams within Office, Division, Department, and with community partners.

Qualifications

Required:

  • Knowledge of and understanding of developmental screening tools
  • Knowledge of and understanding of appropriate developmental milestones for children from infancy through age eight
  • Previous experience working with diverse populations
  • Professional experience using an electronic medical/health record platform
  • Professional database experience
  • Professional social media experience
  • A comprehensive understanding of social services, medical, behavioral health, educational programs, and public entitlements; with a basic knowledge of eligibility and service provision within these areas

Preferred:

  • Ability to speak, read, and write Spanish fluently
  • A degree or equivalent experience in early childhood education, social work, public policy or nursing

Additional Information

  • This position will be based out of the Multi-Agency State Office Building (MASOB) which is located at 195 N 1950 W, Salt Lake City, UT 84116, USA.
  • Risks found in the typical office setting, which is adequately lighted, heated and ventilated, e.g., safe use of office equipment, avoiding trips and falls, observing fire regulations, etc.
  • Typically, the employee may sit comfortably to perform the work; however, there may be some walking; standing; bending; carrying light items; driving an automobile, etc. Special physical demands are not required to perform the work.
  • This position is a time limited position (Schedule TL) that may convert to career service (Schedule B) in the future.
