Summary

This job involves working as a behavioral health case manager in a military health facility, where you'll assess patients' needs, create and adjust personalized care plans, and coordinate services to ensure they get effective support while keeping costs down.

Key Requirements

About the Position: This position is located at an assigned Defense Health Agency (DHA)) within the Military Health System (MHS).

Incumbent maintains responsibility for program management and implementation of the Defense Health Agency System of Care (DHA BHSOC), Behavioral Health Caring Communication / Case Management (BHCCM) Program operations.

Ensures optimal delivery of safe quality, while maximizing resources and containing costs.

Basic Requirement for Behavioral Health Case Manager Degree: Bachelor's or Graduate (or higher level) degree with major study in an academic field related to the medical field, health sciences or allied sciences appropriate to the work of Behavioral Health Case Manager.

In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below: Specialized Experience: One year of specialized experience which includes performing behavior health assessments; providing individual and group support; and assisting patients with access to healthcare systems and resources.

This definition of specialized experience is typical of work performed at the next lower grade/level position in the federal service (GS-11).

Some federal jobs allow you to substitute your education for the required experience in order to qualify.

For the GS-12, you must meet the qualification requirement using experience alone--no substitution of education for experience is permitted. Major Duties:

  • Serves as expert behavioral health case manager and member of multidisciplinary team of medical and professionals to identify high risk and high-cost diagnosis.
  • Perform difficult and complex assessments; conduct interviews; evaluate and analyze health care plan and resources; and plan, coordinate, and monitor the delivery of services.
  • Facilitate optimal patient outcomes by aligning treatment plans with best practices and the unique needs of each patient, providing individual and group support, psychoeducation, and skills training on topics such as stress management.
  • Design timeline-based care protocols that ensure high-quality, cost-effective healthcare for patients and beneficiaries under case management.
  • Tracks client progress towards goals, modifies care plans as needed, and advocates for clients' rights and preferences.
  • Develop, implement, communicate, and coordinate individualized empirically informed patient treatment plans that address all diagnosed conditions and health concerns.
  • Observe and evaluate patient behavior and responses, updating treatment plans in consultation with the healthcare team.
  • Monitor psychotropic medication use, tracks physical and emotional side effects, recommends adjustments to therapy modalities, appropriate level of care placement, and provide clinical feedback to providers.
