Base salary range: $147,649 - $221,900

Note: Actual salary includes locality pay (15-40%+ depending on location).
This job involves helping older adults in Wayne County by assessing their needs, managing their care plans, and investigating protective services cases to keep them safe and independent.
You'll work closely with families, professionals, and community resources while handling paperwork and reports.

The Aging Care Manager 2 receives formal and informal training as required to become knowledgeable in agency rules and regulations.
In this vital role, you will be assigned intake and assessment duties, ongoing care management responsibilities and specialized duties to include investigation of Older Adult Protective Services cases.
You can expect to explain availability, eligibility and restrictions of Agency programs to consumers, family members or professionals and work in close cooperation with community resources.
This position prepares case histories, completes reports and meets all deadlines.
It will be essential to become proficient with agency computer systems and procedures and to attend trainings and informational meetings to remain apprised of program changes.
