The Norfolk Community Services Board (NCSB) provides treatment, support, and education to the community with a focus on mental illness, substance use addiction, and intellectual/developmental disabilities. Norfolk CSB provides services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing in the community.
Today, the NCSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. The NCSB is seeking a Support Coordinator (Case Manager I) in the Developmental & Youth Services division. The Support Coordinator assists individuals with Intellectual and Developmental Disabilities in accessing needed medical, psychiatric, social, educational, vocational, residential, and other supports essential for living in the community and in developing his or her desired lifestyle.Department Hiring Salary Range: $45,444 - $52,148
Requirements
Essential functions include but are not limited to:
- Completes initial and annual assessment of individuals for planning services and supports.
- Develops Individual Support Plan based on assessment information in collaboration with the individual and/or family.
- Provides linkage, coordination, and monitoring of services/supports to include Medicaid Waiver Services provided by private providers, other community agencies, natural support systems and other units in the NCSB.
- Maintains chart documentation that is in compliance with agency guidelines and state regulations. Ensures that documentation substantiates billing for services.
- Conducts face to face visits at a minimum of every 90 days and completes monthly activity related to the individual service plan.
- Monitors progress, service delivery, and individual satisfaction by providing monthly service to all individuals on assigned caseload. Provides increased frequency of service whenever clinically indicated.
- Identifies and accesses resources to meet identified needs and assist individuals in procuring and maintaining resources and entitlements.
- Assists individual/family in obtaining needed services on an emergency basis, i.e. hospitalization, housing, food, medications, etc.
- This position is designated as non-essential for general operations but is required to staff an emergency shelter on a rotational basis. You must maintain open communication with your supervisor, remain available during your assigned shift, and complete both initial and annual shelter training.
- Performs other related duties as assigned.
Qualifications
Bachelor's degree in a human services field, or equivalent combination of training and experience.

Additional Information
- Valid Driver's License may be required depending on assignment.
- Certification in basic first aid and CPR, depending on area of assignment.
Working Hours: Monday-Friday 8:30 AM — 5:00 PM
