The LA County Board of Supervisors recently
declared a local state of emergency for homelessness, and the County is working to revise and expand our response to help all who are affected. We are looking for qualified and passionate individuals to help us in the mission of addressing issues like housing, mental health, and substance use. The Department of Homeless Services and Housing is seeking qualified candidates to fill emergency Supervising Community Health Worker vacancies related to the homelessness crisis.
Under the emergency order, applicants who meet the requirements may be hired for an initial period of up to 90 days, with an opportunity for permanent County employment.
During your initial work period, you will be assessed on your work performance. This assessment will be weighted 100%.
Those who successfully pass the assessment will be considered for permanent appointment to Supervising Community Health Worker.
Requirements
Field Based Engagement and Supervision
- Provide direct, hands-on supervision in the field, including coaching and modeling of trauma-informed, person-centered outreach practices.
- Monitor outreach effectiveness and identify emerging trends, service gaps, barriers, and community needs.
- Communicate field insights, challenges, and recommendations to department leadership to inform program improvements.
Crisis Response & Coordination
- Coordinate and deploy outreach teams in response to urgent and emergent field situations, including medical, behavioral health, and public safety concerns.
- Collaborate with outreach partners, namely LASD and LACoFD, multidisciplinary teams (MDTs), and County departments to ensure timely, appropriate, and coordinated interventions.
Direct Supervision
- Provide direct supervision to outreach staff, including conducting documented biweekly one-on-one meetings focused on performance, development, and support needs.
- Monitor staff performance metrics and ensure alignment with Countywide standards, contract requirements, and program goals.
- Deliver ongoing coaching, mentorship, and corrective action as needed to support staff success and accountability.
- Ensure staff have access to necessary tools, equipment, and supplies to perform their duties effectively.
- Model best practices in outreach, engagement, and documentation.
Encampment Resolution and Field Operations
- Participate in and support encampment resolution efforts in accordance with County protocols and guidelines.
- Coordinate pre-engagement activities, including planning, outreach strategy development, and collaboration with service providers and partner departments.
- Provide field based supervision and support to CHWs and Senior staff during encampment operations and follow-up activities.
- Oversee the development, maintenance, and implementation of By-Name Lists to track and prioritize individuals for services and housing interventions.
Documentation Data Entry and Reporting
- Ensure staff complete accurate, timely, and compliant documentation in HMIS and other required data systems.
- Monitor and track outreach contacts, referrals, service connections, and client outcomes.
- Support performance monitoring, data quality assurance, and compliance with funding and regulatory requirements.
- Prepare and deliver regular reports and status updates on outreach activities and outcomes to leadership.
- Oversee the completion and maintenance of site-specific By-Name Lists, particularly in support of emergency response efforts.

Requirements to Qualify
Option I: Three (3) years of experience at the level of* Senior Community Health Worker** or similar experience at a public or private community-based organization, clinic, hospital or health system.
Option II: Five (5) years of experience at the level of* a Community Health Worker*** at a public or private community-based organization, clinic, hospital, public health organization or health system.

**Experience at the level of Senior Community Health Worker is defined as assisting in the planning, development, implementation, and evaluation of community-based programs and program resources designed to assist clients/patients with physical health, mental health, and social adjustment related issues.
***Experience at the level of Community Health Worker is defined as assisting professional staff in providing direct health, mental health, or social services to clients and patients.
License
A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
Physical Class
Physical Class III – Moderate: This class requires that the incumbent stand or walk most of the time with bending, stooping, squatting, twisting, reaching, working on irregular surfaces, occasional lifting of objects weighing over 25 pounds, and frequent lifting of 10-25 pounds.
Desirable Qualifications
Experience working for a public homelessness services agency providing direct field-based services, such as LAHSA, within the last 6 months.
