Job Description

Summary

This job involves working as a social worker or counselor in a nursing home for Colorado veterans, where you'll assess residents' emotional and social needs, create care plans, provide counseling, and connect them with community resources to improve their well-being.

Key Requirements

  • Four (4) years of relevant experience in social work, counseling, or related fields, or a combination of education and experience totaling four years

  • Proficiency in completing assessments like PHQ-9 depression screens, BIMS cognitive assessments, and MDS 3.0 evaluations quarterly or as needed

Colorado Veterans Community Living Center (VCLC) at Fitzsimons is a beautiful 180-bed, skilled nursing facility serving Colorado’s veterans. We strive to honor and respect those living in our community through companionship and person-centered care while providing a comfortable and safe place to live. At Fitzsimons, we provide long-term care and short-term rehab and have a secure memory care neighborhood. We are conveniently located on the medical campus with UC Hospital and the Aurora VA Medical Center. Fitzsimons celebrates a low 14% staff turnover rate and substantial longevity among staff.


Requirements

While in this position, you are required to work at your assigned work location during your scheduled work times. Please note: Work arrangements are subject to change at any time.

Social Workers shall identify, plan, care for and meet the identified emotional and social needs of each resident to enhance resident psycho-social health and well-being. This position supports the Social Services Director by providing dynamic, comprehensive, and integrated care to meet residents’ physical, emotional, and mental health needs, ensuring psychosocial well-being through assessments, individualized care plans, program coordination, advocacy, and compliance with applicable regulations, while serving as a key resource linking residents, families, staff, and the community.

Job Duties:
  • Interviews, gathers and compiles background data to complete social history/assessment, treatment planning and implementation for delivery of social services to residents, families and significant others.
  • Completes PHQ-9 depression screen and BIMS cognitive assessment quarterly.
  • Completes MDS 3.0 assessment initially, quarterly and at change of condition.
  • Performs assessment for special situations such as suicide ideation, elopement, safe smoking.
  • Assess resident's needs, concerns, issues and remarks using active listening.
  • Identifies solutions in the form of counseling, therapy or social services interventions for resident, through supervision and consultation.
  • Provides education and counseling to decrease any personal, emotional or general concerns that might limit or reduce full utilization of services within the facility.
  • Makes referrals to hospice services, psychological counseling services, ancillary services and other community resources such as diabetic footwear, handicap-parking pass etc. as needed.
  • Discharge planning
  • Attends meetings with interdisciplinary team, as needed
  • Other duties assigned

Qualifications

Minimum Qualifications:

Experience Only:

Four (4) years of relevant experience in an occupation related to the work assigned to this position.


OR

Education and Experience:
A combination of related education and/or relevant experience in an occupation related to the work assigned equal to four (4) years.

AND

Special Qualifications:
Undergraduate Degree in Social Services required by CMS, VA and CDPHE


Preferred Qualifications:
  • Experience working in social work
  • Training relating to mental health and/or trauma
  • Experience as a veteran or working with a veteran population

FULL BACKGROUND CHECK: This position will have direct contact with vulnerable persons (as defined by C.R.S.27-90-111: "Direct contact" means providing face-to-face care, training, supervision, counseling, consultation, or medication assistance to vulnerable persons, regardless of the level of supervision of the department employee.

"Direct contact" may include positions in which persons have access to or unsupervised time with clients or patients, including but not limited to maintenance personnel, housekeeping staff, kitchen staff, and security personnel.)

    • This position will also provide direct care to vulnerable adults requiring a CAPS check (as defined by C.R.S. 26-3.1-101, position provides services & support care to at-risk/vulnerable adults, including case management, clergy, physical or mental health services, safety or welfare services
  • This position requires State of Colorado residency at the time of application (unless otherwise identified in the posting), and residency within the state throughout the duration of employment in this position.

  • Must possess a valid, non-restricted Colorado Driver’s License or a non-restricted U.S. Driver's License for positions within 30 miles of the CO state border

  • Must complete and pass TB test and any other vaccinations as required by the facility.
