Requirements


While in this position, you are required to work at your assigned work location during your scheduled work times. Please note: Work arrangements are subject to change at any time.

This position provides leadership and direction for the Social Services Department, serving as the department manager and supervising Social Services Assistants (SSA). It ensures integrated, continuous care to support residents’ physical, emotional, and mental health, addressing nursing home adjustment and psychosocial needs.

Additionally, as the Director if Social Services you will
  • Provide management to and oversight of Social Services team members in carrying out their day-to-day responsibilities
  • Provides psychosocial, social service expertise, analysis and judgment to staff and other department staff in dealing with the complications that arise in treating residents with medical and psychiatric diagnoses and needs, and by interfacing with numerous multidisciplinary service providers or the residents families or legal representatives.
  • Prepares a report and action plan on a monthly basis on various resident issues that pertain to the social service department for the Quality Assurance/Performance Improvement Committee (QAPI), and attends this meeting conducted by the facility NHA and/or Medical Director.
  • Provides regular feedback to social service staff on their performance and complete PMP performance reviews on an annual basis, or more frequently if necessary.
  • Provides documentation to support recommended corrective and disciplinary actions, and resolves informal grievances
  • Attend meetings with the social services team to ensure that the psychosocial processes, policies, procedures and regulations are being followed.
  • Adhere to facility policies and procedures as well as regulatory standards to provide care and services to the residents of the facility through case management, assessments, care planning/documentation and crisis intervention/conflict resolution to ensure high-quality social services for residents in long term care/domiciliary settings
  • Attend and function as an active participant of the interdisciplinary team by providing reports, in various meetings required by VA, CMS, or other regulatory or facility processes or procedures
  • Complete comprehensive audits of residents charts and files to ensure that accurate information is contained, ensure advance directives are current and uploaded to the EHR, ensure that ancillary services and visits have been completed and uploaded, mental health provider visits are completed and uploaded, ensure that assessments and corresponding notes are documented in the EHR.

Qualifications

Experience Only:
Eight (8) years of relevant experience in an occupation related to the work assigned to this position

OR

Education and Experience:
A combination of related education and/or relevant experience in an occupation related to the work assigned equal to eight (8) years.

AND

Special Qualifications Required:
Licensed Clinical Social Worker required by CMS, VA and CDPHE

CONDITIONS OF EMPLOYMENT:

FULL BACKGROUND CHECK: This position will have direct contact with vulnerable persons (as defined by C.R.S.27-90-111: "Direct contact" means providing face-to-face care, training, supervision, counseling, consultation, or medication assistance to vulnerable persons, regardless of the level of supervision of the department employee.

"Direct contact" may include positions in which persons have access to or unsupervised time with clients or patients, including but not limited to maintenance personnel, housekeeping staff, kitchen staff, and security personnel.)

  • Full background checks include:
    • CBI & FBI fingerprint check, Colorado Court Database check, Medicare Fraud Database, TRAILS Database check.
    • This position will also provide direct care to vulnerable adults requiring a CAPS check (as defined by C.R.S. 26-3.1-101, position provides services & support care to at-risk/vulnerable adults including case management, clergy, physical or mental health services, safety or welfare services)
  • This position requires State of Colorado residency at the time of application (unless otherwise identified in the posting), and residency within the state throughout the duration of employment in this position.
  • Must possess a current and valid Licensed Clinical Social Worker license for the duration of your employment

  • Must possess a valid, non-restricted Colorado Driver’s License
  • Out-of-State Driver’s License holders must obtain a valid, non-restricted Colorado Driver’s License within 30 days of employment start date.
  • Must complete and pass TB test and any other vaccinations as required by the facility.
