Full Job Description

James A. Haley Veterans' Hospital is looking for a Senior Social Worker for Pasco County community-based outpatient clinic who is assigned to the HUD-VASH program.

This position provides Case Management and Housing Search Duties to Homeless Veterans.

JAHVH is a Level I-A tertiary medical center with general medicine, surgical, psychiatric, rehabilitation, spinal cord injury, nursing home care beds, spinal cord injury/disorders center, and a polytrauma treatment facility.

Applicants pending the completion of educational, or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).

Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).

Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited.

A doctoral degree in social work may NOT be substituted for the master's degree in social work.

Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.

May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates.GS-12 Senior Social Worker candidates must have the following: Experience/Education: The candidate must have at least two (2) years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one (1) year must be equivalent to the GS-11 grade level.

Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice.

Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.

Licensure/Certification: Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.

Applicants with Florida LCSW, to qualify for APL Qualified Supervision Delegation, QSCSW must be on licensure.

Senior HUD-VASH Social Workers are capable of meeting all KSAs for GS 11 social work qualification standards 3.

Assignments with Function and Scope For all assignments above the full performance level, the higher- level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time.

Senior HUD-VASH Social Workers are licensed or certified to independently practice social work at an advanced level.

Senior HUD-VASH Social Workers are required to make independent decisions on home visits and while seeing clients in the community to evaluate for their needs.

The Senior HUD-VASH Social Worker may be assigned administrative responsibility for clinical program development and is accountable for clinical program effectiveness and modification of service patterns.

Assignments include clinical settings where they have limited access to onsite supervision such as CBOCs or satellite outpatient clinics.

The senior social worker collaborates with the other members of the treatment team in the provision of comprehensive health care services to Veterans, ensures equity of access, service, and benefits to this population, ensures the care provided is of the highest quality.

4. Social Work Service Expectations within Function and Scope a. Adhere to Service Line Agreements, (SLA) Standard Operating Procedures (SOP) for documentation.

Adhere to approved templates and required standards for documentation as outlined by accrediting bodies such as Joint Commission, CARF, and/or revenue sources providing funding to Veterans Health Administration (VHA).

b.

Provide documentation sufficient to meet the needs of various interdisciplinary teams engaged in the discharge planning process both within the facility and for potential placement and/or aftercare programs/facilities.

c.

Adhere to SLA, SOP, and directives for clinical contact with patients, families, and collateral providers over the course of treatment, which includes pre-admission, program participation, and post-discharge follow-up.

d.

Provide individualize care coordination and discharge planning highlighting patient's strengths, needs, abilities, and preferences within patient interaction, Interdisciplinary Team (IDT) meetings, along with family and collateral contacts.

e. Advise supervisor and IDT on identified treatment goals, priorities, and disposition options and progress. f.

Ensuring patients have access to care via completion of required documents, 1010s, Advanced Directive, and other forms that assist with access to care within the VA or the community. g.

The incumbent will participate in survey accreditation(s) ensuring standards are adhered to within their social work practice, unit/service and at the facility level as well (i.e.

infection control and safety). h.

The incumbent will participate in all assigned supervisory meetings to include individual, team huddles, and will participate in, both social work and matrix, staff meetings. i.

For positions that require use of a government vehicle the incumbent must have and maintain a valid driver's license and pass Occupational Health screening.

Work Schedule: Monday through Friday, 7:30 am to 4:00 pm, with rotating tours of duty as needed.
