DOC is seeking a highly motivated and qualified Registered Nurse 3, Care Manager for the Washington Corrections Center for Women (WCCW) located in Gig Harbor, WA.

This position works closely with primary care teams, other DOC clinicians, and facility HS administrators in caring for patients, and ensures care coordination for high needs patients through incarceration and assists reentry staff in bridging them to the community healthcare system after release.

This position supports the mission of the Health Services division by providing necessary, comprehensive, quality health services and promoting offender wellness.

Requirements

Duties to include:

Ensure continuity and coordination of care for patients identified as medically complex.

  • Collaborates with reentry nurse and psych social worker to ensure continuity of care for high complexity patients after release, to include clinical communication and health information transfer with patient’s community PCP, ensuring post-release appointments in the community are made, and a post-release self-management is in place.
  • Ensures that continuity of care at intra-system transfer by direction communication with the receiving facility’s primary care team and/or care coordinator as applicable.
  • Ensure continuity of care after hospital discharge or emergency department evaluation.

Provides direct patient care and monitoring to the facility’s high complexity patient population.

  • Performs initial comprehensive nursing assessments on patients newly referred for care management, including collection and review of subjective and objective data.
  • Evaluates patients post discharge from hospitalization or emergency department evaluation.

Develops and implements a care plan for facility’s identified as high complexity patient population in collaboration with primary care team.

  • Synthesizes available objective and subjective medical data to create a person and condition specific plan of care upon evaluation.

Collaborates with primary care team, other clinical disciplines, and HS administrative staff to care for the facility’s identified high complexity patient population.

  • Receives and evaluates referrals from primary care team and other Health Services staff for enrollment in the DOC HS Care Management program.
  • Works in conjunction with Patient Services Representative (PSR), Patient Navigator, and primary care Medical Assistant to ensure completion and follow up on offsite referrals.
  • Collaborates with primary care practitioners in management of patient’s chronic and acute illnesses, including communicating any changes in the patient’s status.
  • Collaborates with other clinical disciplines as indicated including behavioral health, dental and SARU.
  • Participates in scheduled multidisciplinary team meetings with the primary care team, Facility Medical Director, and other clinical disciplines to review the status of the complex patient population as needed.

Participates in system-wide development of DOC HS Care Management program

Qualifications

Required Qualifications:
  • Initial and continuous unencumbered licensure in the State of Washington as a Registered Nurse
OR
  • An unencumbered Multistate License from a participating Compact Licensure State as outlined by the Nurse Licensure Compact and Department of Health with completion of Suicide Prevention, EDIR training, and MLS demographic survey.
AND
  • Two years' experience working as a Registered Nurse
  • Training by the American Heart Association in Healthcare Provider CPR.
  • Proficient using computer-based programs to schedule, monitor and track patient information, perform data entry and word processing, and communicate via email, as well as other programs that may be needed to complete assigned work.
  • Ability to work independently and with limited supervision.

Graduation from an accredited school of professional Nursing

  • Must be able to work in spaces, climb stairs, walk long distances, and frequently lift up to 30 pounds. Under rare circumstances this position may be required to lift or carry up to 50 pounds and push up to 100 pounds.
