Work Unit Description
The Compliance and Innovation Division is responsible for ensuring the Office of Community Living supports efforts and processes assisting members moving from an institutional setting into a community setting and diverting members from more restrictive settings through education and resource support and connection.
The In-Reach Unit exists to support individuals in need of Long Term Services and Supports for transitioning into the community from an institutional setting and to assist with diversion into an institutional setting through education and referrals to supporting agencies.
The unit oversees Group In-Reach, Individual In-Reach, Rapid Reintegration, At-Risk/Diversion projects and other OCL program functions, as necessary.
This unit works in coordination with other divisions to align with parallel and ongoing efforts to ensure that programming is consistent with all state and federal policy guidance.
It is the goal of the In-Reach Unit to support Medicaid organizations that help individuals access adequate care, regardless of age, disability or geographic region.
The Unit plays an important role in working toward the Department’s mission to improve access to cost-effective, quality long-term care services and support for Coloradans.
The Regional In-Reach Specialist is responsible for supporting individuals residing in Nursing Facilities (NFs) by ensuring awareness of Home and Community-Based Services (HCBS) and facilitating successful transitions to community settings.
This position leads In-Reach, training, technical assistance, and stakeholder coordination efforts to promote person-centered practices and compliance with Department of Justice (DOJ) agreement requirements.
This role also provides escalation support, resolves complex transition barriers, and contributes to program performance, quality improvement, and data tracking efforts statewide.
Provides support to residents, family and staff in Nursing Facility (NF) to ensure understanding of Home and Community Based Services and other community living options.
Consult with NF staff to provide guidance on creating a consistent understanding of the process across facilities, and support knowledge gaps through turnover to ensure person-centered processes.
Consult with NF residents through In-Reach activities, educating them on transition services and community resources available to residents.
Collaborates with facilities to ensure that residents have access to independent referral sources. This position ensures that there is a referral station accessible in each facility providing referral sources in multiple modalities, local contact information, and resource guides.
Implements or attends resident council meetings.
Distributes materials to facilities to provide contact information and overview of community-based resources.
Ensures that current contact information is visibly posted in each facility, and instructs residents how to access this information.
Completes Group and Individual In-Reach activities as outlined in the voluntary DOJ agreement Position is a resource to NF staff for technical assistance and ongoing communication.
Works with NF administration and staff to implement internal training to be managed by NF staff, with quality oversight provided by this position.
Completes all tasks outlined in the LCA contracts for the Regions without Contractors.
Coordinates referrals to Case Management Agencies (CMAs), RAEs, Local Contact Agencies (LCAs), and Transition Coordination Agencies (TCAs).
Investigates barriers to transition including eligibility, housing, service authorization, discharge delays, and inter-agency coordination issues.
Collects and analyzes documentation related to escalated cases.
Determines appropriate corrective actions and required follow-up timelines based on established processes.
Coordinates resolution across multiple entities to ensure timely discharge.
Experience Only:
Six (6) years of professional experience supporting individuals with disabilities or older adults, coordinating across stakeholders to resolve complex service issues, and/or providing training and presentations, with knowledge of Medicaid, and/or other community-based services.
OR
Education and Experience:
Bachelor's Degree in Public Policy, Health Care Services, Social Services, or related field, and two (2) years of professional experience supporting individuals with disabilities or older adults, coordinating across stakeholders, resolving complex service issues, and/or providing training and presentations, using knowledge of Medicaid, and/or other community-based services.Preferred Qualifications
Experience with educating members on Colorado Medicaid Long-Term Services and Supports (LTSS) programs and Home and Community-Based Services
Experience supporting individuals transitioning from an institutional setting to the community
Experience resolving complex escalation, complaint resolution, or complex case management capacity involving multiple stakeholders.
Lives in South Central Colorado - Alamosa, Conejos, Costilla, Mineral, Rio Grande, Saguache Counties
Knowledge of adult learning principles demonstrated through education, training, or certification
Ability to analyze data and trends to inform decision-making and improve program outcomes
Strong understanding of person-centered practices and community integration principles
