About the work unit
This work unit exists to assure that there is agency-wide compliance with all agency, State, and Federal laws, rules and regulations to maintain Medicaid funding to support the health and welfare and active treatment of all individuals served by this agency within the Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID). The Quality Assurance (QA) Department safeguards and advocates for the human/civil/legal rights of the people served; and assures the development and implementation of agency operating instructions and the training of these procedures. This work unit also evaluates the systems in place in the agency to ensure the health and safety and protection of the rights of persons served.

About the Position:
The purpose of this position is to assure that the agency is in compliance with all agency, department, state and federal laws, rules, and regulations that apply to individuals of this agency in protection of their human/civil/legal rights and the development and implementation of the highest quality of services for persons served. The position participates in the agency’s investigation processes for all reports of critical incidents, serious injuries, abuse, neglect, and mistreatment. This position also trains the Rights of Persons Served to agency personnel. Additionally, the purpose of this position is to perform program quality functions, which ensures program integrity and prepares the organization for licensure and recertifications surveys.

Primary Job Duties:

Investigator. Independently conducts investigations in compliance with Labor Relations Alternatives standards and State, and Federal rules and regulations regarding violation of rights of persons served: writes analytical, factual reports related to this process; is responsible for investigative practices including notification to regulatory agencies and law enforcement agencies and interprets guidelines to meet reporting requirements to the Colorado Department of Public Health and Environment, Colorado Department of Human Services, Adult Protections Services, and law enforcement. Reviews recommendations of investigations with the management team for their development of action plans.

Incident Reports. This position is responsible to manage, monitor, review, audit and analyze data in the General Event Review (GER) incident reporting system. If an incident report requires follow-up, the position ensures that follow-up is assigned and completed by appropriate staff members. Position mails copies of incident reports to appropriate contacts and ensures that incident reports are filed accurately and in a timely manner. Position is the expert on the incident report database. Position maintains the database and all information related to data entry. The position is responsible to ensure that all incident reports are entered and all follow up is entered as well.

Position is responsible for conducting training for new employees on the GER incident reporting system. Position is responsible for reviewing monthly and year-to-date data in iNotify to develop reports for use in trending and analysis to identify problems areas and issues for the development of recommendations and solutions.

Program Quality. The position is responsible for collaborating with QAPI presenters to evaluate the relevance of data collection theories and concepts, tailoring them to develop specialized reporting plans for monthly presentations. Beyond standard data entry, the position analyzes complicated data sets to break problems into parts, interpreting findings through the lens of professional standards to design individualized tables, charts, and data summaries. The position conducts advanced trend analysis, using judgment and resourcefulness to identify the root causes of occurrences where established precedents may be unclear or inadequate. Consequently, the position originates and establishes unique action items and corrective plans tailored to specific circumstances to ensure the continuous quality and care of residents.
The position manages and directs the monthly QAPI framework by designing the schedule and workflow for complex data integration. Rather than providing standard suggestions, the position uses judgment and resourcefulness to tailor data collection models and trend-tracking theories to fit the unique clinical needs of the facility, especially where general precedents are non-specific. The position is responsible for originating and updating individualized presentation materials and systems to reflect outcomes where regulations or policies may be unclear.

Position evaluates the relevance of complicated monthly and year-to-date data, breaking the information into parts to reach conclusions that result in new work processes. By drawing analogies from professional standards and concepts, the position identifies unprecedented problem areas and develops a different approach or plan to formulate recommendations and solutions for specific circumstances that standard guidelines do not cover.

Position is responsible for completing audits of data as assigned by the Quality Assurance department and reporting/inputting the data into numerous databases for use by the appropriate PRC departments

Position tracks and maintains materials and results by utilizing Excel spreadsheet.

HIPAA Rules & Regulations. Position will serve as the PRC authority on Health Insurance Portability and Accountability Act (HIPAA) rules and regulations. Responsible for developing, implementing, and maintaining HIPAA policies and procedures and assures compliance with federal law. Develops and conducts educational and training programs on HIPAA for the workforce. Manages, investigates, resolves and reports on non-compliance allegations including contact with patients and workforce and provides recommendations of sanctions and mitigation to the management team. Maintains and generates a weekly compliance report on all HIPAA complaints and compliance issues including resolution.

Training. Establishes standards and evaluates effectiveness of training to professionals and para-professionals on the protection of the rights of persons served and prevention and reporting responsibilities for abuse, mistreatment, neglect, and exploitation, as well as prevention of workplace violence.

Qualifications


Experience Only:

Six (6) years of relevant experience in an occupation related to the work assigned to this position

OR

Education and Experience:

A combination of related education and/or relevant experience in an occupation related to the work assigned equal to six (6) years.


Preferred Qualifications:

  • Professional investigative and/or compliance experience in a regulatory environment.

  • Experience conducting investigations and/or audits.

  • Experience working with datasets and conducting analyses.

  • Experience utilizing Microsoft Office Suite and Google Suite.

  • Experience working with confidential records to ensure HIPAA compliance.

  • Experience working with individuals with intellectual and developmental disabilities (IDD).

  • Good Work History

  • Relevant years of State service experience

  • Travel – Position requires travel between remote worksites to conduct investigations. Must utilize state fleet vehicles, if available. If not, must utilize a personal vehicle and request mileage reimbursement.

  • Must demonstrate the ability to lift 50 lbs, bend, squat, etc. and perform behavioral interventions proficiently (training and continuously throughout employment).
