Bruce McCandless Veterans Community Living Center
The Bruce McCandless Veterans Community Living Center (VCLC) at Florence offers holistic skilled nursing care for veterans, surviving spouses, and Gold Star parents, who need short-term rehabilitation, respite, or long-term care. The facility focuses on providing individualized, respectful care in a safe and comfortable environment. The VCLCs identifies and addresses residents’ diverse needs from admissions, transfers, and discharges, including social, emotional, psychological, and medical assistance. It advocates for residents, removes barriers, coordinates services, and addresses concerns. The unit also facilitates interactions between residents, families, staff, and external resources and ensures proper documentation for reimbursements.

Requirements

This is a full-time, in-person, essential services designated position that requires a CNA licensure through the Colorado Department of Regulatory Agencies. This position may be used to fill day or evening positions.


Please note: These working arrangements are subject to change.
This position exists to provide direct hands-on resident care by hands on performance or assistance as needed with activities of daily living. Provide environmental needs by arranging and adapting personal care items such as food or clothing and/or physical environment needs, e.g. wheelchair, walker, etc. Gently guide and instruct residents as needed in self-care activities.

Primary Job Duties Include:

Resident Care
  • Provide direct resident care by hands-on performance or assistance as needed with activities of daily living.
  • Provide environmental needs by arranging and adapting personal care items such as food or clothing and/or physical environment needs, e.g. wheelchair, walker, etc.
  • Gently guide and instruct residents as needed in self-care activities.
  • Assists, orients, and serves as a preceptor to new CNAs to the facility, residents, policies and procedures, and routines.
  • The CCA II is a resource to the CCA I for suggestions and guidance in dealing with care for resistive residents.
  • Determine changes in the resident's type of care required; i.e., variance in resident's individualized routine care due to change in resident's condition or observation in health status.
Transportation, Education and Coaching

This position provides and acts as a mentor to newly hired and assigned Client Care Aide I as part of the new hire orientation training.

Transport residents for medical appointments, deliver blood draws to the lab, drive a resident to a doctor’s appointment, or to the hospital (if not transported by ambulance). etc.

Maintain education by attending mandatory and non-mandatory in-house and off-site educational training, cornerstone online training and staff meetings to maintain current on techniques, policies to keep current as mandated by state law, VA regulations and facility policy and procedures.

MINIMUM QUALIFICATIONS:

Education/Licensure/Certification/Experience:
High school diploma or General Educational Diploma (GED) AND One year of experience in an occupational field related to the work assigned to the
position

AND

Must have a valid Colorado CNA license upon entry into the position

Employment history is calculated on a full-time basis (40/hrs per week). Part-time employment is calculated on a prorated basis to determine qualifications. Substitutions:

Current, valid nurse aide license (CNA) from the Colorado Department of Regulatory Agencies will substitute for the high school diploma or GED.

Current, valid licensure as a Practical Nurse (LPN) from the Colorado Department of Regulatory Agencies or any state participating in the Nurse Licensure Compact will substitute for the experience requirement.

Required Competencies
  • Skill in initiating, performing, and completing assigned duties in providing care to residents.
  • Possess good verbal, listening, and written communication skills with all health care providers, residents, families, and multidisciplinary team members.
  • Demonstrated ability to display professional conduct at all times when dealing with fellow staff members, residents, families, and the general public.
  • Demonstrated ability to maintain confidentiality at all times.
  • Long-term care/nursing home experience is highly desirable.
  • Effective problem-solving skills.
Preferred Competencies
  • Knowledge and ability to appropriately carry out assigned resident care based on the residents' conditions; to use judgment and to accurately recognize report, and record relevant resident information.
  • Knowledge and ability to explain assigned resident care best practices to peers with less experience.
  • Knowledge and ability to recognize urgent or emergent resident care situations, seek guidance and direction of the RN and/or Supervisor/DON, and initiate appropriate emergency interventions as directed.
  • Knowledge of quality standards, procedures, and protocols.
  • Knowledge of specialized client care applicable to the work situation.
  • Ability to effectively work with a resident population of varying physical, mental, and emotional abilities and staff from multiple work units in the facility to achieve quality care planning and delivery.
