Pay Schedule III, Grade 18N, Regular Schedule: 35 hours per week.
A vacancy exists in the Department of Health and Human Services, Community Health Services, Cancer Prevention Program.

Requirements
Under general supervision, plans, coordinates, and provides professional public health assessment and care coordination for screening and diagnostic services related to cancer testing for individuals and providers in Baltimore County.
Essential Job Duties
- Provides nursing assessment, care planning and referral services to individuals, and institutions.
- Reviews clinical history and demographic information for clients upon assignment of cases.
- Contacts assigned clients/representatives in accordance with state and county policies.
- Coordinates with clients and providers to ensure appropriate and timely screening and follow up.
- Maintains professional relationships with providers.
- Ensures provider acknowledgement and compliance with state clinical guidelines when necessary.
- Provides case management, ongoing evaluation, and education of client care plan.
- Maintains accurate and detailed documentation in electronic health record and program databases
- Independently organizes and prioritizes workload to meet program deadlines and performance metrics.
- Assesses client need for community-based resources in accordance with the social determinants of health.
- Ensures compliance with state, federal and Medicaid regulations.
- Provides health screening education and counseling in the areas of disease prevention, health maintenance, and mental health.
- Participates in the instruction and orientation of new public health nurses.
- Prepares and maintains nursing records and reports.

Qualifications
Required Qualifications
Graduation from an accredited college or university with a bachelor's degree in nursing.
OR
Graduation from an accredited diploma school of nursing;
Plus
At least one year of professional nursing experience. (Additional qualifying education may be substituted on a year-for-year basis for the required experience.)
OR
Graduation from an accredited college or university with an associate in arts degree in nursing:
Plus
At least two years' professional nursing experience.
Additional qualifying education may be substituted on a year-for-year basis for the required experience.

- Experience in public health or community-based programs.

Licenses and Certificates
Possession of a valid license to practice as a professional registered nurse, issued by the Maryland State Board of Examiners of Nurses, is required at the time of appointment. Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment.
