Job Description

Summary

This job involves working with families and children through the CHANT program at Metro Nashville's Health Department, where you'll assess needs, connect families to health resources, and support them in homes or community settings to improve overall well-being.

It's a hands-on role that requires building trust with clients, planning services, and handling challenging cases.

  • Bachelor’s degree and 5 years of professional experience in home visiting, service coordination, assessing families for resources, and early intervention

  • Identify and secure medical, behavioral health, and dental services, including health coverage through TennCare/Medicaid or other plans

  • Perform administrative tasks such as documentation, data entry, and reporting; participate in trainings and team meetings
  • Valid driver’s license, personal vehicle, and insurance; provide culturally and linguistically appropriate services

Full Job Description

The Chant programs is seeking a highly motivated program specialist. This position will evaluate referrals received by CHANT Team Lead for appropriate next step and action planning. Will engage client-families assigned by CHANT Team Lead in a variety of settings such as the clients’ home, at a community event or community center, or by client contact with Lentz (MPHD).

Requirements

Evaluate referrals received by CHANT Team Lead for appropriate next step and action planning.Participate in inter-professional team meetings to provide opportunities to share ideas, share best practices and develop comprehensive service plans Engages client-families assigned by CHANT Team Lead in a variety of settings such as the clients’ home, at a community event or community center, or by client contact with Lentz (MPHD).Conducts an initial “screening” assessment to determine if program/coverage thresholds may be met.Identifies the clients’ needs based upon the current “condition” of the family.Provide developmentally appropriate education and resources for parents/guardians.Identifies medical, behavioral health and dental medical homes for child(ren)/families and seeks to ensure health coverage for targeted child(ren)/families through TennCare/Medicaid, CoverKids or Affordable Care Health Plans.Assists client-families with navigating selected “Pathways of Care” which helps the family set goals for the child/children/family to make improvements to their overall health.Identifies appropriate referrals for the family based upon its needs as well as available MPHD and community resources.Develops community partnerships to promote awareness of services and increase resource capacity.Plan, conduct, and participate in community-based events.Educates client-families using evidence-based and developmentally appropriate guidance.Performs general administrative duties including accurate documentation in client records, statistical reports, data entry, emails and letters.Participate in CHANT related continuing education trainings, webinars, conference calls and other professional development opportunities offered at the state and local level to maintain expertise.Follow Children’s Special Services (CSS) policy for children eligible for medical services.Provide information to customer that is culturally and linguistically appropriate to meet the needs of the population being served.Manage assigned caseloads with a higher degree of responsibility, requiring specialized, technical, skill sets and intense problem resolution through navigation.Performs other duties as assigned to support the Bureau of Population Health and advance the mission of the Metro Public Health Department Qualifications Any combination of education and experience that would prepare the incumbent to perform the duties of the position at the appropriate level.

A Program Specialist 2 for the CHANT program would typically have a Bachelor’s and 5 years’ professional experience in home visiting, service coordination, assessing individuals and families for resource delivery and early intervention and will be expected to handle high-intensity cases.

Valid driver’s license, use of personal vehicle, and maintenance of valid personal vehicle insurance as required by Tennessee Law.
