Summary

This job involves working as a nurse who helps coordinate and manage healthcare for military patients, acting as a bridge between them and doctors across military and civilian systems.

You'll assess patients' needs, create care plans, handle referrals, and update records to ensure smooth treatment.

About the Position: Incumbent serves as a Nurse (Clinical/Case Manager) responsible for analyzing problems associated with providing health care to patients and/or beneficiaries for the Military Treatment Facility (MTF).

This position is at Patuxent River. This is a Direct Hire Solicitation Who May Apply: US Citizens In order to qualify, you must meet the education and/or experience requirements described below.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social).

You will receive credit for all qualifying experience, including volunteer experience.

Your resume must clearly describe your relevant experience; if qualifying based on education, your transcripts will be required as part of your application.

Education/Experience: A diploma, associate degree, or bachelor's degree in Nursing from a professional nursing program and possession of at least one full year of professional nursing experience (equivalent to the GS-9 grade level or above) which includes, 1) coordinate healthcare services at Military Treatment Facilities 2) serve as patient's liaison with healthcare professional across military and civilian sectors 3) complete follow up assessments in the electronic record with updated information of the patient's current condition.

OR Education: A PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.

EXEMPTION: You are exempt from the qualification requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before.

Note: You will be required to provide appropriate documentation to the respective Human Resources Office to validate your status. Major Duties:

  • Coordinate healthcare services available at Military Treatment Facility (MTF).
  • Conduct a comprehensive assessment of beneficiary’s health needs to develop a plan of care.
  • Serve as liaison between the patient and the health care professionals.
  • Coordinate referrals for each patient under case management services.
  • Assess patients with various complex medical conditions daily.
  • Complete follow up assessments in the electronic record with updated information of the patient's current physical and emotional condition.
  • Responsible for the integration of Utilization Management, Case Management, and Disease Management based on the Military Health Systems (MHS) Medical Management Model.
  • Serve as the first level supervisor performing supervisory responsibilities.
