About the Position: This position is with the Defense Health Agency, Defense Health Network East at Martin Army Community Hospital located within the Department of Mental Health at Fort Benning, Georgia.

Basic Requirement for Mental Health Case Manager: Degree: Bachelor's or graduate (or higher level) degree with major study in an academic field related to the medical field, health sciences or allied sciences appropriate to the work of the position.

This degree must be from an educational program accredited by an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.

In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below: Specialized Experience: One year of specialized experience which includes providing direct case management services, developing care plans and identifying patient needs to ensure continuity of care for patients with complex needs; and reviewing procedures or processes to recommend improvements to healthcare access, administrative processes, or other efficiencies within a medical facility.

This definition of specialized experience is typical of work performed at the next lower grade/level position in the federal service (GS -11).

Some federal jobs allow you to substitute your education for the required experience in order to qualify.

For this job, you must meet the qualification requirement using experience alone--no substitution of education for experience is permitted. Major Duties:

  • Serve as expert mental health case manager and member of multidisciplinary team of medical and professionals to identify high risk and high-cost diagnosis.
  • Develop, implement, communicate and coordinate individualized empirically informed patient treatment plans that address all diagnosed conditions and health concerns.
  • Communicate with treatment facilities, both in direct and purchased care to coordinate patient care and transitions. -Communicate/coordinate with appropriate parties including command and other medical teams within the guidance provided by DHA.
  • Track client progress towards goals, modify care plans as needed, and advocate for clients' rights and preferences.
  • Coordinate meetings to ensure treatment plans for at-risk and high-interest patients are synchronized among all medical, Substance Use Disorder Clinical Care, Family Advocacy Program, and all other medical treatment providers.
  • Monitor psychotropic medication use, track physical and emotional side effects, recommend adjustments to therapy modalities, appropriate level of care placement, and provide clinical feedback to providers.
  • Schedule and monitor medical appointments, therapy sessions, and support services and ensure continuity of care across multiple systems.
  • Conduct regular resource and community needs assessments in order to be more informed about needs of client base.
