About the Position: Responsible for program management and implementation of the Defense Health Agency System of Care (DHA MHSOC), Mental Health Caring Communication / Case Management (MHCCM) Program operations for 48th Medical Group Lakenheath Air Base.

Position is located at Lakenheath, United Kingdom.

Basic Requirement for Mental Health Case Manager: Degree: Bachelor's or graduate (or higher level) degree with major study in an academic field related to the medical field, health sciences or allied sciences appropriate to the work of the position.

This degree must be from an educational program accredited by an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.

In addition to meeting the basic requirement above, to qualify for this position you MUST also meet the qualification requirements listed below: Specialized Experience: One year of specialized experience which includes mental health case management; coordinating mental health care plans with a multidisciplinary team; and evaluating appropriateness, quality, or cost effectiveness of care.

This definition of specialized experience is typical of work performed at the next lower grade/level position in the federal service (GS-11).

For this job, you must meet the qualification requirement using experience alone--no substitution of education for experience is permitted. Major Duties:

  • Serve as expert mental health case manager and member of multidisciplinary team of medical and professionals to identify high risk and high-cost diagnosis.
  • Perform difficult and complex assessments; conduct interviews; evaluate and analyze health care plan and resources; and plan, coordinate, and monitor the delivery of services.
  • Track client progress towards goals, modifies care plans as needed, and advocates for clients' rights and preferences.
  • Design timeline-based care protocols that ensure high ­quality, cost-effective healthcare for patients and beneficiaries under case management.
  • Develop, implement, communicate, and coordinate individualized empirically informed patient treatment plans that address all diagnosed conditions and health concerns.
  • Work with mental health clinic staff to ensure patient needs are met based upon the patient's assessed condition.
  • Chair and coordinate At-Risk/Multi-Disciplinary Treatment Planning (ARCT/MDTP) meetings.
  • Assist patients navigate healthcare systems and access necessary resources.
  • Manage program resources effectively, ensuring that they are used in a fiscally responsible and ethical manner.
