PUEBLO REGIONAL CENTER (PRC)
This work unit exists to provide a safe, secure, therapeutic setting for comprehensive/holistic care of individuals with both developmental disabilities and medical issues to include evaluation and treatment. This includes 24-hour nursing care for individuals. The emphasis is providing outstanding support and services while offering a wide range of treatment options. The focus of treatment is stabilization, increased knowledge base, increased knowledge of treatment options (i.e., medications), individual and group therapy, discharge planning, working with families, support systems for individuals, and an understanding of developmental disabilities, health/medical issues. Individuals with developmental disabilities, physical handicaps, and multiple medical diagnoses also have multiple needs in addition to mental health stabilization are treated on this at their home. Individuals are admitted for the purpose of evaluation, observation, and treatment. The unit goal is to return them to their optimal level of functioning and assist them in returning to the community as appropriate. All individuals are empowered to choose in what manner they will participate in the life of the community.

Requirements

About the Position

To ensure that personal outcomes are identified for people receiving support and services, and that with planning process attainment of outcomes is attempted in a systematic and person-centered way, utilizing existing (and to be realized) resources to the maximum.

Compliance With Agency Requirements.

Advocates and oversees processes to support personal outcomes, documentation, staff activities to confirm compliance to Person’s requirements, federal standards; Division Regulations, agency policy and procedures, and to preserve funding resources.

Monitors delivery of services to assure constant habilitation training occurs throughout the day through formal and informal methods.

Observations recorded using Monitor database and other recording systems, while program compliance and follow-through evaluated through periodic file reviews.

Outcome.

Annually (or as needed) completes the Person Focused Worksheet to discover preferred Outcomes of the people receiving supports and services.

Facilitate as needed to ensure development of needed support and services to help the person obtain their desired Outcomes. Co-facilitate and/or write the Individualized

  • Plan process with the Case Manager assigned
  • Adaptive Daily Living Skills.
    • Assesses, develops, revises, and delegates the implementation of individual adaptive daily living skill programs.
    • Effectiveness is evaluated by direct observation; input from Team members, and the person’s responses to programming efforts.
    Appointing Authority.
    • Provide direct supervision and appointing authority to Health Care Tech IVs in the provision of support for obtaining outcomes relating to work, recreation, leisure, and money management.
    • Provide second level supervision for up to 22 para-professional FTEs assigned to the living area through direct and delegated authority.
    • This also includes general oversight of several units, including environment, budget, and response to licensure deficiencies
    Training.
    • Assures through direct or delegated authority the training of employees in the therapeutic strategies and techniques as required for compliance to written programs, rules and regulations, policy and procedure, or internal operating.
    Service and Support.
    • Provide hands-on support for people and staff as needed to ensure continuity of services and supports and to assist
    Duty Officer.
    • Be available for agency Duty Officer, to consult with staff after hours.
    • Responsible for making administrative decisions and intervention decisions for people receiving support and services as well as staff.
    Other Duties as Assigned.
    • Will need to drive to various group homes, will take or assist people on outings, to court dates or other appointments.
    • Travel between group homes and transport individuals as needed.
    • Performs other duties as assigned.

    Qualifications

    Experience Only:

    Eight (8) years of relevant experience in an occupation related to the work assigned to this position

    OR

    Education and Experience:

    A combination of related education and/or relevant experience in an occupation related to the work assigned equal to eight (8) years.

    Preferred Qualifications:

    • Experience supervising staff to include operating in a leadership capacity, providing performance evaluations, hiring, and providing progressive discipline.
    • Experience working with individuals with intellectual and developmental disabilities (IDD).
    • Bachelor’s degree in a human services related field.
    • Experience in HCBS-DD Waiver implementation.
    • Experience in program management.
    • Good work history.
    • Relevant years of state service experience.
    Conditions of Employment.
    • Full Background CDHS employees (all Direct contact with vulnerable persons): CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services),Trails check (direct contact with children), CAPS (direct contact with adults – Mental Health Institutes, Regional Centers, Veterans Community Living Centers).
    • This position requires State of Colorado residency at the time of application (unless otherwise identified in the posting), and residency within the state throughout the duration of employment in this position.
    • Motor Vehicle Check , if the employee will drive a state-owned vehicle, either in an on-going capacity while performing their normal day-to-day job duties OR on occasion for training/meetings/client visits/etc.
    • Must possess a valid, non-restricted Colorado Driver’s License or an non-restricted U.S. Driver's License for positions within 30 miles of the CO state border.
    • Shift Work . Will be required to be in contact with subordinates on all shifts to provide support needed. Will also serve as duty officer to consult with staff after hours. Travel.

      Must travel between group homes and transport individuals as needed. Position is required to use a State vehicle to transport residents.

      When not transporting individuals, this person may drive a State vehicle or choose to drive their own vehicle.

    • Former State employees who were disciplinarily terminated or resigned in lieu of termination must:
