Job Description

Summary

This job involves helping people with developmental disabilities by assessing their health-related social needs, such as access to meals, housing support, or caregiver relief, and connecting them to the right services through referrals and authorizations.

Developmental Disabilities Case/Resource Manager

The Home & Community Living Administration (HCLA) Developmental Disabilities Community Services (DDCS) Division is hiring a Health-Related Social Needs Referral and Authorization Case Manager to work in our Tacoma or Tumwater office.

This project position is working closely with the Health Care Authority (HCA) to administer the array of innovative new Health-Related Social Needs (HRSN) services and supports to include medically tailored meals, food and vegetable prescriptions, pantry restocking, air conditioners and installation for high-risk clients with access to cooling, pest eradication services, displacementhousing funding, hoarding remediation, and caregiver respite services.

The scope of clients HRSN services are available to include all Apple Health eligible clients when deemed medically appropriate using clinical and other health-related social needs. Where you will work

This position is located in ether the Tacoma or Tumwater office, and you have the option to work in office, telework at your home, or we also support a hybrid work model of office and telework.

The standard work schedule is Monday – Friday, 8am – 5pm. You may have the option of a modified work schedule after successfully completing your 6-month probationary period.

You will be required to attend training outside of your local office. Travel within the region to conduct in-person assessments and meetings with clients and community partners will be required.

Peirce County employees are eligible to receive an Orca Card for transportation needs.

This position is covered by a collective bargaining agreement.

Some of what you will do:

  • Conduct functional and financial HRSN assessments using the department’s Comprehensive Assessment Reporting and Evaluation (CARE Web) to determine level of care and eligibility for the HRSN
  • The HRSN assessment will be completed meeting the quality standards set forth in the Long-Term Care Manual
  • Complete initial client assessments and person-centered service plans in CARE within established timelines as required for caseload and according to WAC and send appropriate notices to client and representative
  • Identify support needs of clients of DDCS and plan for and authorize payment for appropriate services
  • Obtain translation and interpretation services for clients and families when needed or requested
  • Based on assessed needs, refer individuals to appropriate DDCS services, other department services, and community programs/resources
  • Identify clients or families in crisis and make emergent referrals to appropriate authorities, department/administration services, state plan services, or local community resources

We are looking for professionals who have:

A Bachelor's degree in social sciences, social services, human services, behavioral sciences or an allied field and two years of experience providing social services to people with intellectual or developmental disabilities.

Or six years of experience in providing social services to people with intellectual or developmental disabilities.

Or Satisfactory completion of 12 months as a Case Resource Manager Trainee.

Or Equivalent experience/education

Graduate training in the areas listed above will substitute, year for year, for one year of the experience providing social services to people with developmental disabilities.

Preferred knowledge, skills, and abilities:
  • Possess basic computer skills, including use of e-mail, basic software, data entry, and internet access
  • Working knowledge of the Developmental Disabilities Community Services Division residential and county service guidelines, RCWs, WACs, and policies/procedures
  • Ability to safeguard sensitive and confidential information and documents, and maintain confidentiality regarding client information, yet identify issues of a serious and/or sensitive nature that need to be reported to the supervisor
  • Able to prioritize and organize work in a fast-paced work environment
  • Demonstrate sensitivity to cultural diversity
  • A Certified interpreter allows for a 5% pay increase but is not required
