This case manager position serves as the primary case manager for individuals who have a mental illness or intellectual disability and who are involved in the adult criminal justice system.

Requirements
The case manager is responsible to advocate and assist consumers in gaining access to needed medical, social, educational, and other services. The outcome of these services will focus on integration in the community, vocational/educational participation, and adequate social supports to successfully complete the conditions of their probation. The case manager functions in a team modality to determine the least intrusive and least restrictive method that can be employed in providing the treatment necessary to manage symptoms of mental illness and behavior that result in recovery and wellness and provide protection for the individual served, their family, and the community. This is a highly responsible supportive counseling/case management position in the Forensic Unit.
Responsibilities include developing individualized service plans; reviewing the consumers' receipt of and participation in services; reviewing clinical assessment information; assisting consumers in establishing informal support networks; assisting consumers in identifying, accessing, and learning to use community resources; submitting necessary documentation; and other duties as assigned.
Work Schedule and Additional Information:- Full time employment, 8:30 am through 4:30 pm, Monday-Friday, with a 30 minute lunch.

Qualifications
QUALIFICATIONS
Minimum Experience and Training Requirements:
- Six months of experience as a County Caseworker 1; or
- Successful completion of the County Social Casework Intern program; or
- A bachelor's degree with a social welfare major; or
- A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency; or
- Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
- Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.

- You must pass a background investigation.
- This position falls under the provisions of the Child Protective Services Law.
- Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
