Job Description

Summary

This job involves helping people in the community with health-related social services, such as providing information, making referrals, managing cases, and conducting outreach for disease prevention and screening in public places.

It includes tasks like drawing blood, transporting clients, and visiting homes to deliver education and medications.

A good fit would be someone compassionate with experience working with the public, comfortable in outreach settings, and able to handle physical tasks like lifting and driving.

Provide social service information, referrals, clerical support and follow-up case management services for persons in the community seeking assistance. Assist clients applying for public services.

Requirements

1. Provides social service information, referrals and follow up case management
2. Will identify clients through street outreach and solicitation at various public and private venues for disease risk reduction messages and screening opportunities
3. May contact parties for drug screens, call lab, call attorneys with results and keep files
4. May transport clients seeking service to the health department
5. Will perform venipuncture for clients seeking service at various venues in the county
6. Screens clients to determine immediate needs and makes proper referrals according to protocols
7. May perform outreach and educational functions in the community
8. May be required to make home visits as part of case management and client education which includes: driving to public or private client locations, dispensing health education information, delivering/administering medications under strict protocols and obtaining a completed general health questionnaire from clients
9. Documents all outreach activities and client services: pulls court jackets, searches records, contacts attorneys and prepares files for warehouse
10. May act as reception desk relief at noon and in the absence of the receptionist answering and phones and greeting clients
11. Performs all other related duties as assigned

Qualifications

  • High school diploma or GED with one (1) year of experience working with the public. Additional education may be substituted for experience on a year for year basis.
  • Must have the ability to read, write, and comprehend the English language.
  • Must have a valid driver's license.

Additional Information

While performing the duties of this position, the incumbent is regularly required to stoop, bend, push and pull. May have to lift heavy boxes or objects up to 50 pounds and perform outdoor setup of tables and tents for screening activities.
