The Norfolk Community Services Board (NCSB) provides treatment, support, and education to the community with a focus on mental illness, substance use disorders, intellectual/developmental disabilities, and housing and homelessness. Norfolk CSB provides services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing in the community.

Today, the NCSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. The NCSB is seeking a Community Liaison (Case Manager IV) to serve as the central point of entry for DD Services for Norfolk residents. Assesses eligibility for DD Waiver services, completes intake assessments for Support Coordination services, and provides information and external referrals as indicated. The liaison delivers Developmental Services information as directed by DOJ agreement and monitors individuals awaiting DD Waiver services. A key part of component of this position is to provide outreach to local community partners such as public school, advocacy groups, colleges, community centers, etc. to educate on DD Services and identify individuals in need. Serves as liaison in coordinating discharge planning activities for Norfolk residents receiving care at State operated training centers and community Intermediate Care Facilities for Individuals with Intellectual Disabilities. Provides specialized case management service for a small caseload of individuals with co-occurring DD, Behavioral Health, and/or Substance Use. Provides case management and related services in various human service areas. In conjunction with program supervisors, may assist with supervision of small staff.

Department Hiring Salary Range: $56,900-$65,294

Requirements

Essential functions include but are not limited to:
  • Completes individual service plans to determine identified need areas and requests for services. The meeting must be done in face-to-face contact either through visits to the home, office, or other community setting.
  • Prepares Individualized Services Plan based on assessment information in collaboration with individual and/or family.
  • Provides assessment, advocacy, linkage, coordination, and monitoring for services to include Medicaid Waiver.
  • Services provided by private providers, other community agencies, natural support systems and other units within the NCSB.
  • Complies with DBHDS, DMAS, and licensure regulations and maintains chart documentation that complies with agency guidelines.
  • Monitors progress, service delivery, health and safety, and client satisfaction by providing monthly service to all individuals on assigned caseload. Provides increased frequency of service whenever clinically indicated.
  • Identifies and accesses resources to meet identified needs and assist individuals in procuring and maintaining resources and entitlements.
  • Provides information to consumers, significant others and services providers regarding client rights and promptly reports any suspected violation of those rights to supervisor and Adult or Child Protective Services (APS or CPS).
  • Complies with regulations for HIPPA, mandated reporting of abuse and neglect, Client Confidentiality, and other Client Rights.
  • Assess eligibility for DD Waiver services and conduct the VIDES assessment.
  • Provide monitoring services for individuals on the DD Waiver Wait list for Norfolk.
  • Provide information and external referrals to individuals seeking services.
  • Conduct presentations regarding DD Waiver and DD Services to local community groups.
  • Liaison with local agencies to promote DD education and awareness of services available.
  • Provide information to those individuals that may qualify for Developmental Disabilities services.
  • Attend treatment planning meetings and collaborates with Social Workers for individuals residing in institutional settings.
  • Works with family or significant others in the development of discharge plans to maximize natural supports and facilitates transition/discharge process for facility individuals that are designated as ready for discharge from institutional care to the community.
  • Monitors individuals residing in facilities through collateral contacts with family, social workers, and Community Integration Managers. Assess individuals at least annually face to face. Monitor progress, maintain documentation, review annual and other reports and participate in meetings.
  • Implement the pre-discharge plan and ensure all the necessary linkages to community support and providers of services.
  • Provides supervision to staff assigned monthly and as needed for meetings, performance appraisal, and performance management, quality review, on the job training, and guidance and counseling.
  • Attend staff meetings, prescription team, in-services, and conferences as required.
  • All other duties assigned by the Programs Manager.

Qualifications

Bachelor's degree in a human services field.

Considerable experience in a human services field related to the area of assignment. Some assignments may also require specific experience working with various types of client populations; master's degree preferred in some areas.

Additional Information

  • Valid Driver's License may be required depending on assignment.
  • Certification in basic first aid and CPR, depending on area of assignment.

Working Hours: Monday-Friday 8:30 AM-5:00 PM
