Summary

This role involves leading a team of pharmacists focused on anticoagulation therapy, such as managing blood-thinning medications, while spending most of your time providing direct patient care in a veterans' hospital.

Full Job Description

The Anticoagulation Program Manager coordinates the daily work of anticoagulation clinical pharmacy staff, participates in all Pharmacy Service activities including clinically oriented programs, regulatory and quality assurance activities, and coordinates all functions and activities within the section to ensure they coincide with other sections and the overall goals and mission of Pharmacy Service.

The Anticoagulation Program Manager maintains an active clinical practice 80% of their time.

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.

(NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program.

Today the sole degree is Doctor of Pharmacy.) (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).

Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.

The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.

A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.

May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Preferred Experience: Experience managing anticoagulation therapy, including warfarin and direct oral anticoagulants (DOACs), with demonstrated knowledge of pharmacokinetics, monitoring parameters, and dose adjustments.

Experience with tracking clinical outcomes, adverse events, and compliance metrics; ability to use data for performance improvement.

Ability to mentor staff, provide education on anticoagulation best practices, and lead process improvement projects.

Grade Determinations: In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. GS-13 (a) Experience.

Candidates at this grade level are to be in one of the assignments listed below.

For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time.

Clinical Pharmacy Specialist The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients.

A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services.

This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy.

Pharmacists assigned to this position must demonstrate the following KSAs: a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b.

Expert understanding of regulatory and quality standards for their program area. c.

Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d.

Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e.

Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

2) Develops facility policy related to use of anticoagulants encompassing policies governing practice within the anticoagulation management program and other practice areas of VANIHCS, including surgical or specialty areas.

3) Promotes learning and evidence-based practice for the anticoagulation management program through delivery and coordination of educational activities, regular staff meetings, and journal clubs.

4) Develops quality assurance criteria, monitors and evaluates quality of anticoagulation care provided by VANIHCS and reports results to appropriate facility governing body, i.e.

Pharmacy and Therapeutics Committee.

5) Responsible for coordination of VANIHCS Anticoagulation Clinic staff which includes reviewing consults, providing patient care, and assuring appropriate coverage for the clinic.

6) Ensures compliance of the Anticoagulation Management Program with all regulatory and accrediting body requirements, i.e. The Joint Commission, FDA, ASHP, DEA, OIG, etc.

7) Ensures compliance with VHA Directive 1033, Anticoagulation Therapy Management. 8) Communicates process improvement opportunities to staff and coaches staff to implement new process improvements.

9) Organizes workload, directs clinical activities, plans short and long-term objectives/goals, solves problems and achieves established goals within budget and staffing constraints.

10)Incorporates performance improvement (PI) activities into all areas of the clinical pharmacy section under the incumbent's oversight.

11) Assists with the effective utilization of Pharmacy Service resources to support the mission and goals of the organization.

Clinical 1) Maintains an active clinical practice within area of expertise which might include reviewing consults, providing direct patient care, and assuring coverage for clinical services when on leave.

2) As a provider in area of clinical practice expertise, evaluates patients and monitors response to medication therapy.

Makes changes to medication therapy within scope of practice as needed depending on assessment of appropriate laboratory and clinical monitoring.

Also recommends patients for referral, when presenting with other medical signs and symptoms, to appropriate clinic and/or provider.

3) Educates patients on their medication therapy, including but not limited to medication compliance, dietary restrictions, and patient monitoring parameters.

4) Reviews medication regimens within area of clinical practice and either takes action or notifies appropriate provider of any findings that might require action, including but not limited to: i.

Misuse of medication, i.e., abuse, overuse, or underuse ii. Inappropriate dose, dose interval, or dosage form iii. Drug and/or pharmacologic duplication iv.

Adverse drug-drug, drug-lab, or drug-food interactions v. Adverse drug reactions vi. Suboptimal response vii.

Suboptimal therapeutic regimen 5) Initiates alternative drug therapies designed to reduce cost of medications while still ensuring optimal therapeutic response.

6) Advises patients and providers on aspects of clinical pharmacokinetics as it pertains to the monitoring of serum plasma concentrations and its relationship to patient response.

Also, advises providers how to individualize dosing of selected medications based on interpretation of these serum levels and evidenced based care.

Education 1) Coordinates educational activities for clinical staff directly involved in caring for patients receiving anticoagulation therapy, i.e.

clinical pharmacists, clinical pharmacy specialists, pharmacy technicians, registered dietitians, nurses, APRNs, physician assistants, and physicians.

2) Proactively provides ongoing communication related to the pharmacy programs under their supervision to professional staff via Pharmacy and Therapeutics Committee meetings, Medical Staff meetings, Pharmacy Department meetings and other meetings, as appropriate.

3) Maintain competency in best practices, medication management, and other areas related to the clinical area of supervision.

4) Supports Chief of Pharmacy and Chief of Staff in aligning VA providers to support organizational stewardship.
