The Cincinnati VA Medical Center is looking for a Licensed Pharmacy Service Chief who will have the responsibility to plan, direct, and coordinate comprehensive pharmacy services in a complex, affiliated Medical Center which includes separate facilities, Community Based Outpatient Clinics, and access centers located in Cincinnati, OH, Southwest Ohio, Southeast Indiana, and Northern Kentucky.
Basic Requirements Citizenship. Citizen of the United States.
(Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.) English Language Proficiency.
Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
Education (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.

(NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program.
Today the sole degree is Doctor of Pharmacy.) (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet[1]Based Test (TOEFL iBT).
Licensure Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.
A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
1.Exception. Non-licensed pharmacists who otherwise meet the eligibility requirements may be given a temporary appointment at the entry level as a Graduate Pharmacist under the authority of 38 U.S.C.
§ 7405(c)(2)(B). The appointing official may waive the requirement of licensure for a period not to exceed 2 years for a pharmacist that provides care under the supervision of a licensed pharmacist.
For grade levels above the GS-11, the candidate must be licensed. 2.Failure to Obtain License.
In all cases, pharmacists must actively pursue meeting state prerequisites for licensure starting from the date of their appointment.
At the time of appointment, the supervisor will provide the unlicensed pharmacist with the written requirement to obtain licensure, the date by which the license must be acquired, and the consequences for not becoming licensed by the deadline.
Failure to become licensed within 2 years from date of appointment will result in removal from the GS-0660 Pharmacist series and may result in termination of employment.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: GS-15 1. Service Chief.
Responsible for all aspects of pharmacy services at a medical facility by maintaining effective interdepartmental relationships demonstrating active problem resolution skills and close cooperation with other programs and services in accomplishing the medical facility mission and goals.
Responsible for budget management, determining resource needs, allocating resources, ensuring proper utilization in productivity, efficiency, and cost effectiveness of the operation.
Formulates objectives and priorities and implements plans consistent with the long term interest of the organization, capitalizing on opportunities and managing risks.
Responsible for compliance with all safety, regulatory, and accrediting body requirements. Delegates authority to several supervisory and program levels to meet the goals of the service and staff.
Ensures orientation and training programs are established to ensure staff development, oversees academic affiliations, and participates in medical facility, VISN, National, other external governmental agencies, and professional associations.
(a) Experience. In addition to the GS-14 requirements, must have 1 year of experience equivalent to the next lower grade level. (b) Grade Level Criteria.
At the GS-15 level, Chiefs of Pharmacy must meet the following: 1. Individuals are assigned as Chiefs of Pharmacy at Complexity Level 1a, 1b or 1c VHA facilities; or 2.
Individuals are assigned as Chief of Pharmacy at Complexity Levels 2 or 3 and must meet the criteria noted in 2a and b below. a.
Must meet all of the following requirements: (1) Manages a complex medical center based pharmacy program that is defined as having multiple practice settings (e.g.
hospital ambulatory care, long-term care, behavior health care) that require Joint Commission review as separate standards.
(2) In addition to the medical center pharmacy, has responsibility for pharmacy services at one or more of the following: CBOCs, Rural Health Program, domiciliary, or provides dispensing and/or clinical services to State Veterans Home.
(3) Presence of advanced clinical pharmacy programs that include pharmacist(s) that have approved scopes of practice with prescriptive privileges.
(4) Pharmacy provides clinical pharmacist services to three (3) or more specialty areas such as, but not limited to, hemodialysis, oncology, operating room or an emergency department. b.
In addition to the requirements listed in 2a above, must meet at least two of the following: (1) An ASHP Accredited Pharmacy Residency Program.
(2) A Research Program that is accredited by the Association for the Accreditation of Human Research Protection Programs (AAHRPP).
(3) Actively involved in committees or advisory/work groups at the VISN and/or National level.
(4) An ongoing pharmacy student rotation in both the intermediate and advanced practice experience settings. (c) Assignments.
Candidates at this grade level are to be in one of the assignments listed below.
For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time.
(d) Demonstrated KSAs. The KSAs shown below apply to all assignments at this grade level, and the specific KSAs designated by an asterisk (*) apply to assignment 1 and 2. 1.
Skill in persuading others and gaining cooperation to accomplish goals. 2. Ability to effectively communicate orally and in writing regarding complex clinical and technical issues. 3.
Skill in utilizing regulatory and quality standards to develop and implement operational programs. 4. Skill in managing multiple people or programs. *5.
Skill in utilizing available resources to support the missions and goals of the organization. *6. Ability to plan, organize and direct the functions of the pharmacy staff.

The Cincinnati VA Medical Center (CVAMC) is a two-division campus located in Cincinnati, OH and Fort Thomas, KY serving 15 counties in Southwest OH, Northern Kentucky, and Southeast Indiana with 6 Community Based Outpatient Clinics located in Bellevue, KY; Florence, KY; Lawrenceburg, IN; Hamilton, OH; Clermont County, OH, and Georgetown, OH.
The CVAMC also supports two off main campus locations in the Cincinnati area via an Eye Center and Mental Health Outreach Community Division. Duties: a.
Administration/Supervision: Plans, directs, and coordinates Pharmacy Services.
Recruits, interviews, and selects suitable personnel, in collaboration with selection committees and Human Resources, in order to maintain a staff that can effectively fulfill the needs of the Service.
Recommends individuals for appointment, promotion, and reassignment involving such positions.
Attends, participates, and contributes as an active member of various Medical Center committees: Pharmacy and Therapeutics Committee, Clinical Executive Board, Affiliation Partnership Council, Administrative Executive Board, etc.
Initiates and demonstrates activities to support Equal Employment Opportunity. Prepares, manages, and forecasts the budget for the service.
Optimally use resources to meet the identified goals of the organization.
Analyzes and corresponds to local, VISN, and PBM/VACO officials regarding the status of pharmaceutical services to professional staff and Veteran beneficiaries.
Formulates and prioritizes long and short-term goals for the Service and coordinates these with the goals of the Medical Center, VISN, and VHA utilizing available resources.
Coordinates activities of the pharmacy supervisors to organize work to maximize the productivity within available resources.
Assures that direct reports are scheduled in such a way as to provide consistency of coverage and to accommodate planned leave and promote employee morale. Periodically analyzes work measurement data.
Controls and accounts for all regulated substances in the Pharmacy in accordance with station policy, VA, and DEA regulations.
This responsibility includes establishing and maintaining reports and participating in inspections.
Provides advice, counsel, and instruction to individual supervisors and employees, concerning both work and administrative matters. Resolves conflicts and problems referred by supervisors.
Works with other services to complete goals and improve communications.
Teaches and guides supervisory staff in solving problems; adapting to procedural changes and implementing new technological changes.
Ensures that pharmacy programs are in compliance with the standards of accrediting and regulatory bodies and takes action when non-compliances are identified.
Manages and ensures optimum utilization of all resources (personnel, fiscal, equipment, space, etc.) available to the pharmacy.
Manages and ensures optimum efficiency and effectiveness of the numerous, complex, and comprehensive programs administered by the Pharmacy Service.
Manages optimal pharmaceutical care, such that it supports the Medical Center and VISN's efforts to meet Performance Measure expectations for quality of care, access to care, customer satisfaction and employee satisfaction.
b. Clinical: Develops and maintains effective clinical pharmacy programs from a cost and patient outcome perspective.
Serves as a consultant within Pharmacy Service, and with other facility healthcare staff in monitoring and evaluating healthcare delivery to patients.
Assesses the pharmacy program to ensure proper coordination between the delivery of pharmacy services and the overall delivery of healthcare. c.
Education and Research: Develops and maintains effective pharmacy education programs, including the ASHP accredited Residency Program and pharmacy student rotations.
Identifies continuing education and training needs based on performance of all subordinate staff in coordination with the VISN 10 Pharmacy Education and Resource Committee.
Serves as preceptor for residents in the ASHP accredited Pharmacy Residency program. Participates in self-development activities to maintain and improve professional competency.
Completes all assigned training by the due date. May design and/or participate in research studies involving drug therapy or clinical pharmacy functions.
Encourages and supports service-related efforts to publish and present in professional journals and at professional meetings respectively.
