Summary

This job involves helping people with developmental disabilities or brain injuries apply for state services, manage their waiting lists, and connect them to community support while they wait.

Caseworker Specialist II


The Division of Services for People with Disabilities (DSPD), within the Department of Health & Human Services, is looking for a dedicated professional to fill our Caseworker Specialist II position.

As a Caseworker Specialist II on the Intake and Waiting List team you will provide support to individuals with Developmental Disabilities or Acquired Brain Injuries going through the application process or on the waiting list for DSPD services.

Support includes: responding to calls and emails from clients, advocates, or families; reviewing documentation to determine if individuals meet program eligibility; updating annual assessments; linking with community resources to help meet needs while on the waiting list; and completing referrals for emergency services when appropriate.

Other essential job duties include attending community events targeted toward individuals with disabilities, participating in multi-agency committees, and presentations to community partners.

Intake and Waiting List Caseworkers have large caseloads and must be able to prioritize tasks, respond to calls and emails within expected time frames, and maintain accurate documentation within their caseload files.

The individuals and families we support may be dealing with difficult circumstances that result in heightened emotions at times and Caseworkers may have to employ techniques to de-escalate discussions.

Requirements

  • Coordinate Assessments and Support for clients.

  • Support Individuals to dictate their service plans

  • Complete field work assignments to complete assessments used to support Persons initial Person Centered Support Plan (PCSP) and budgets; RFS tasks, and UCANS assessments.

  • Provide consultation, make recommendations, give appropriate advice, and/or facilitate decisions.

  • Coordinate and refer services of community resources, placement providers, collateral agencies, the court, and/or other community partners with families, clients, or patients receiving services.

  • Assess client/patient needs. Develops and implements plans to meet those needs.

  • Support the Waiting List Support Team

  • Respond to court orders for restoration training

  • Schedule appointments with clients in the community and/or jail

  • Coordinate transition from Vocational Rehabilitation to Support Employment program on DSPD waitlist

  • Complete field visits to assess needs of the Supported Employment participants

  • Coordinate with contracted DSPD providers, community partners and shareholders to meet the needs of clients in the SWI program

  • Understand how to create Person Centered Support Plan and budget for SWI participants

  • Develop reports and gather information as requested by DSPD leadership

Qualifications

Minimum:
  • Has strong interpersonal skills
  • Is passionate about empowering People with Disabilities
  • Is an excellent listener who is also very proactive
  • Has the ability to work well with a wide range of people with varying interest
  • Has knowledge of community resources for people with disabilities and how to access those resources
Preferred:
  • Current State of Utah employee with the Division of Services for People with Disabilities (DSPD) that feels strongly about advocacy and empowering individuals with disabilities in Utah

  • Has a Bachelor's Degree or 5 years of relevant experience or a combination of education and experience.
  • Has obtained certification as a QIDP (Qualified Intellectual Disability Professional)
  • Has knowledge of Home and Community Based Services (HCBS)

  • Serving as an Caseworker Specialist II involves working alone in clients' homes to support youth and families with complex and unpredictable behaviors. The role requires the ability to confidently and tactfully handle potentially dangerous or stressful situations.
  • Some travel throughout will be required for this position.
  • The candidate selected for this position will either be given access to a state-owned vehicle, or will receive mileage reimbursement for use of a personal vehicle.
  • Due to this position requiring travel via motor vehicle, candidates will be required to have a valid Driver License.
  • Continued employment is contingent upon the successful completion of a fingerprinting background check.
