Job Description
Summary
This job involves helping seniors and adults with disabilities by assessing their needs, creating personalized care plans, and coordinating in-home services to promote their independence.

Key Requirements
- Master’s Degree in behavioral or health sciences plus 1 year of paid social service experience, or Bachelor’s Degree in similar fields plus 2 years of paid social service experience, or Bachelor’s Degree plus 4 years of paid social service experience

CASE MANAGER for Olympic Area Agency on Aging (O3A) based in Forks, WA.
Requirements
Provides case management for in-home care to seniors and adults with disabilities including assessment of care needs, creating service plans, coordinating care providers, and focusing on client independence.
Qualifications
Master’s Degree in behavioral or health sciences & 1 yr. paid social service experience or BA in similar fields and 2 yrs. paid social service experience; or BA and 4 yrs. paid social service experience.
Requires current WDL & insured vehicle.
Additional Information
40 hrs./wk. Non-Exempt.
$70,453 to $92,441 annually.
