The Luzerne-Wyoming County Area Agency on Aging (AAA) is seeking a compassionate and dedicated Aging Case Aide 2 to join the team.

As the Aging Case Aide 2, the majority of your work will be with the Intake/Assessment Unit covering Luzerne and Wyoming Counties.
In this role, it is essential you maintain on-going proficiency in the use of the statewide Department of Aging databases known as Well-Sky and PIA.
Work includes contacting consumers by telephone, assisting in the screening of referrals for Assessment, and referring consumer information to the appropriate department.
You will have the opportunity to field all incoming telephone calls from consumers who have questions about Aging Programs such as Needs Assessments, Functional Eligibility Determinations, home modifications, OPTIONS, or other programs.
You can expect to complete request for assistance forms for consumers who come into the agency as walk-ins and conduct home visits in the community to those consumers who require assistance in gathering financial documentation or other application information.
