This job involves helping older adults aged 60 and up by assessing their needs, creating personalized care plans, and ensuring they receive the right services to stay safe and independent.
It also includes investigating complaints like abuse or neglect, providing support to staff and partners, and overseeing community programs for seniors.
A good fit would be someone compassionate with experience in social work or related fields, who enjoys problem-solving and working with vulnerable populations.

The Aging Care Manager 3, Department of Human Services (DHS), Area Agency on Aging (AAA), Older Adult Support Bureau, Care Management Unit, under the general direction of the Casework Supervisor, is responsible for comprehensively evaluating the needs of older adults, identifying unmet needs and gaps in services, developing service (care) plans and managing the complexity of needs and specialized services for older adults.
The Aging Care Manager 3 is also responsible for monitoring service offerings, programs and wait lists associated with the delivery of service to older adults in the community.
Responsibilities also include care managing, providing consultation, training and case conferencing with contracted care management agencies regarding complex cases and reviewing contracted care management agencies responsiveness to consumer needs and adherence to care management policies, procedures and processed relative to the provision of social services.
Work Schedule and Additional Information:
Telework: You may have the opportunity to work from home (telework) part-time, up to two days per week after six months of employment.
In order to telework, you must have a securely configured high-speed internet connection and work from an approved location inside Pennsylvania.
If you are unable to telework, you will have the option to report to the headquarters office in Pittsburgh. The ability to telework is subject to change at any time.
