Summary

This job involves working as a care manager to support older and disabled adults in Westmoreland County by assessing their needs, connecting them to services, handling emergencies, and monitoring their care to help them live independently.

  • Full-time availability for 8:30 AM to 4:00 PM Monday-Friday schedule, with possible standby rotation for off-hours coverage
  • No telework option; position is in-person in Pennsylvania

Requirements

As an Aging Care Manager 2 you will obtain pertinent information through direct contacts with consumers, family members, professionals and others in order to evaluate the information and to determine consumer needs and the appropriate course of action.

As the need arises, handles emergency situations employing crisis intervention techniques and linking needy consumers with appropriate help, e.g.

law enforcement, medical and/or mental health professionals or others.

Completes reports-of-need in conjunction with Acts 13, 70 & 79 as defined by PA legislation and regulations, and assigns appropriate categories of response to direct required timely intervention.

You will monitor and provide follow-ups of agency services and make referrals for any changes needed for the consumer.

Incumbents work with different aide staff and other department staff on projects related to referrals, ongoing services, monitoring, follow-ups, and assistance with benefit program application duties.

Additionally, you will assist with providing educational and informational programs to senior groups, civic groups, civic clubs or any interested organizations in Westmoreland County.


Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:30 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
  • May be required to be on a standby rotation for off-hours coverage.
  • Telework: You will not have the option to telework in this position
  • Salary: In some cases, the starting salary may be non-negotiable.
