Provides medication delivery for treatment of TB infection and/or disease, assists patient with initiation of VDOT, and notifies Nurse Case Manager immediately of any missed appointments.
Utilizes incentives and enablers to assist with adherence. Provides groceries and transportation with TB Unit Manager approval.
Participates in Continuous Quality Improvement (CQI) activities such as record reviews, program evaluations, team meetings, etc.
Maintains records and reports in accordance with agency standards including documentation in the Electronic Health Record (EHR), Personnel Cost Accounting System (PCAS), weekly reports, etc.
Conducts Social Work (SW) peer audits as designated.
Assists with agency shelter operations, and client emergency response including First Aid, Cardiopulmonary Resuscitation (CPR), and Automated External Defibrillator (AED), and be willing and available to assist during times of public health emergency and outbreak response efforts.
Other duties as assigned
Basic life support (BLS) / CPR / First Aid certification within thirty (30) days of hire or provide documentation of satisfactory course completion prior to employment
DRIVING RECORD: If this position requires the applicant to possess a valid driver’s license to operate a state vehicle or personal vehicle, any applicant being considered in the final stages of selection for the position will be required to provide a certified copy of a 10-year driving record.
STATE DISASTER PLAN: In accordance with the State's Disaster Plan, which includes hazardous weather, Department of Public Health employees may be required to work in times of an emergency or disaster.
