Job Description

Summary

This job involves helping people facing mental health, substance use, or other challenges by reviewing referrals from emergency responders like police and firefighters, connecting them with support services, and providing short-term help through in-person, phone, or virtual sessions.

It also includes following up on high-risk cases, building ties with law enforcement, and covering shifts across the county.

A good fit would be someone with experience in social work who enjoys crisis intervention, community outreach, and teamwork with first responders.

Key Requirements

  • Master's degree in social work or related field with emphasis on social work methods, human growth, and social welfare principles, OR Bachelor's in social work, psychology, sociology, or human services plus 18 graduate credits and 3+ years supervised social work experience
  • Licensed as a Graduate Social Worker or equivalent (full details in classification spec)
  • Current regular or probationary employee of Hennepin County Human Services and Public Health (internal applicants only)
  • Ability to work on-site in North Minneapolis with travel throughout Hennepin County
  • Availability for Monday-Friday 10 a.m.-6:30 p.m. schedule, including some evenings
  • Experience in crisis intervention, suicide risk assessment, and short-term stabilization services
  • Skills in documentation using state and county systems, community engagement, and cultural competency

Full Job Description

The Behavioral Health area is seeking two Senior Social Workers (SSW) to support early identification and intervention for individuals experiencing behavioral health, mental health, medical, and substance use challenges who come into contact with emergency response systems such as law enforcement, fire, or EMS. You will be a part of the Central Intake team within the Embedded Social Work Program (ESW). In these positions, you will review referrals from first responders, coordinate with existing providers, and conduct outreach to individuals and families referred for services. You will also deliver short-term stabilization services in person, by phone, or virtually, and require follow-up with clients identified as high-risk on the Columbia Suicide Severity Rating Scale. Additionally, these positions will provide coverage as needed for ESW, Alternative Response Team (ART), and Co-Response SSW's assigned to police departments across Hennepin County.


Location and hours:
These positions will be on-site and located at the North Minneapolis Human Service Center at 1001 Plymouth Avenue North, Minneapolis, MN 55411, and will also require travel throughout Hennepin County to provide coverage as needed to various police departments. The work schedule will be Monday through Friday, 10 a.m. - 6:30 p.m., with some limited evening hours.

In these positions, you will:
  • Review referrals from first responders, coordinate with existing providers in the community, and conduct outreach to individuals and families referred for services.
  • Deliver short-term stabilization services in person, by phone, or virtually.
  • Follow-up with clients that have been identified as high-risk on the Columbia Suicide Severity Rating Scale.
  • Provide coverage for ESW, ART, and Co-Response SSW's assigned to police departments across Hennepin County.
  • Build collaborative relationships with law enforcement professionals and department leadership by offering social service-related consults, attending, and actively engaging in precinct meetings, presenting social service-related information at roll calls and, going out on occasional ride along with officers.
  • Assist law enforcement officers in real time to provide engagement, support, and resources to residents.
  • Develop interventions in collaboration with the resident to create a personalized plan that is culturally informed, including coordination with existing supports, providing resources and referrals for ongoing needs.
  • Document referrals received and case notes utilizing state and county systems.
  • Participate in community engagement events.
Need to have:
  • One of the following:
    • Master's degree or higher in social work or closely related field with course work emphasis in areas such as social work methods, human growth and behavior, and principles of social welfare.
    • Bachelor's degree in social work, psychology, sociology, or human services, and at least 18 semester (27 quarter) credits of graduate work in the above areas, and three years or more of full-time supervised experience as a social worker.
    • Licensed as a Graduate Social Worker by the Minnesota Board of Social Work.
  • A valid driver's license, daily access to reliable transportation, and the ability to obtain a Hennepin County Driver's Permit.
Nice to have:
  • Licensed or working towards licensure in any of the following:
    • Licensed Graduate Social Worker (LSGW), Licensed Associate Marriage and Family Therapist (LAMFT), Licensed Professional Counselor (LPC), Licensed Independent Clinical Social Worker (LICSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC).
  • Experience:
    • Providing case management.
    • Serving individuals living with a mental health condition, substance use disorder, and/or homeless/housing instability.
    • Collaborating with law enforcement to address the safety and needs of individuals.
    • Effectively interacting with people from diverse backgrounds including efforts towards disparity reduction.

  • Knowledge of:
    • Diversity of cultural and lifestyle experiences, beliefs and practices relating to spiritual, mental, and physical health, community, family structure and dynamics and disparities.
    • County and community resources for mental health, substance use disorder, social services, and basic needs.
    • Harm reduction principles.
    • Trauma-informed care principles.
    • Person-centered thinking.
    • Social determinants of health.
    • Social work best practices and ethics.

  • Ability to:
    • Function well in unpredictable and challenging situations calling for sensitivity, flexibility, resourcefulness, and independent judgment.
    • Work well independently and as a team.
    • Demonstrate effective interpersonal, organizational and computer skills.
